TO PROMOTE SOCIAL JUSTICE AND THE WELL BEING AND EMPOWERMENT OF ASIAN PACIFIC AMERICAN INDIVIDUALS, FAMILIES, AND COMMUNITIES - INCLUDING IMMIGRANTS, REFUGEES, AND NATIVE BORN - BY DEVELOPING, PROVIDING, AND ADVOCATING FOR INNOVATIVE COMMUNITY-BASED MULTILINGUAL SERVICES.

THE BEHAVIORAL HEALTH (BH) PROGRAM SERVES OVER 2822 INDIVIDUALS AFFECTED BY MENTAL, EMOTIONAL, AND BEHAVIORAL PROBLEMS. THE BH PROGRAM COMBINES EASTERN AND WESTERN APPROACHES TO OFFER A COMPREHENSIVE ARRAY OF SPECIALIZED MENTAL HEALTH SERVICES TO INDIVIDUALS AND FAMILIES WITH A TEAM OF TRAINED MENTAL HEALTH CASE MANAGERS, PSYCHIATRISTS, AND PSYCHIATRIC NURSES THAT SPEAK MORE THAN TWENTY DIFFERENT ASIAN AND PACIFIC ISLANDER LANGUAGES AND DIALECTS. BH STAFF ASSIST INDIVIDUALS TO DEVELOP AND REGAIN THE SKILLS AND INDEPENDENCE TO LIVE MEANINGFUL LIVES IN THEIR COMMUNITIES. BH STAFF ALSO PROVIDE IMPORTANT LINKAGE TO OTHER COMMUNITY RESOURCES INCLUDING HOUSING, PUBLIC BENEFITS, AND SUPPORTIVE EMPLOYMENT. STAFF WORK COLLABORATIVELY WITH HOSPITALS, NURSING HOMES, AND OTHER HEALTH CARE PROVIDERS TO COORDINATE CARE FOR THEIR MUTUAL CLIENTS.
THE AGING AND ADULT SERVICES (AAS) PROGRAM PROVIDED ASSISTANCE IN OBTAINING BASIC SERVICES TO OVER 8,070 LOW-INCOME SENIORS AND ADULTS WITH DISABILITIES. THE CASE MANAGEMENT PROGRAM SERVED OVER 3,800 HOMEBOUND ELDERS AND ADULTS WITH DISABILITIES ENABLING THEM TO SAFELYREMAIN INDEPENDENT AT HOME, NEARER THEIR FAMILIES AND FRIENDS. THE COMMUNITY LIVING CONNECTIONS PROGRAM, FORMERLY INFORMATION AND ASSISTANCE, SUPPORTS THE WELL-BEING OF ASIAN AND PACIFIC ISLANDER ELDERS BY PROVIDING ASSISTANCE AND REFERRAL WITH HEALTH INSURANCE,MEDICARE, MEDICAID, LEGAL ISSUES, TRANSPORTATION, NUTRITION, AND SOCIAL ACTIVITIES. THE HEALTH HOME PROGRAM PROVIDES COORDINATION OF MEDICAL AND SOCIAL NEEDS WITH SERIOUS AND COMPLEX HEALTHCARE CHALLENGES TO HELP SUPPORT ELDERS AND ADULTS WITH DISABILITIES TO MANAGE THEIRONGOING CHRONIC CONDITIONS AND OVERALL WELL-BEING.
NUTRITION - WE DISTRIBUTE HEALTHY AND NUTRITIOUS FOODS LIKE RICE, TOFU, NOODLES AND FRESH PRODUCE THAT MEET THE CULTURAL AND DIETARY NEEDS OF ASIAN AMERICANS AND PACIFIC ISLANDERS. ACRS SERVES NUTRITIOUS ETHNIC MEALS TO OLDER ADULTS WHILE THEY SOCIALIZE, EXERCISE AND ATTEND EDUCATIONAL WORKSHOPS IN THE LANGUAGES THEY SPEAK.
