EXEMPT PURPOSE: TO ASSIST VULNERABLE INDIVIDUALS, ESPECIALLY MEDICALLY FRAGILE OLDER ADULTS AND THOSE WHO ARE DISABLED TO GAIN STABILITY THROUGH EMERGENCY SHELTER,ACCESS TO MEDICAL CARE, SYSTEMS NAVIGATION AND PATHWAYS TO HOUSING.

H4H WORKS DIRECTLY WITH UNHOUSED MEDICALLY FRAGILE OLDER ADULTS AND PEOPLE OF ALL AGES WHO ARE DISABLED AND LIVING ON THE STREETS OF SAN DIEGO. WE MEET THE MAJORITY OF OUR PARTICIPANTS ON THE STREETS OF SAN DIEGO THROUGHOUT THE WEEK AT ALL HOURS OF THE DAY AND NIGHT. H4H PARTICIPANTS ARE OFTEN FOUND DISORIENTED, INADEQUATELY CLOTHED, INCONTINENT, AND UNABLE TO WALK. WE EMPLOY AN INDIVIDUALIZED PERSON-CENTERED TRAUMA-INFORMED APPROACH FOCUSED ON EQUITY, PROGRESSIVE ENGAGEMENT, AND HARM REDUCTION CONNECTING THE INDIVIDUAL WITH AN ADVOCATE CASE MANAGER TO HELP THEM STABILIZE, CONNECT TO SERVICES, AND ULTIMATELY TO A HOUSING SOLUTION.


OUR NURSE LED OUTREACH AND RELIEF BASED OUTREACH TEAMS PROVIDED DIRECT MEDICAL ASSISTANCE AND RELIEF SERVICES AND SUPPLIES TO THOUSANDS OF PEOPLE IN 2023. WE HAVE ALWAYS TRACKED THE PEOPLE WE ENGAGE WITH DURING NURSE-LED OUTREACH, AN AVERAGE OF 40 PEOPLE PER MONTH BASED ON AN AVERAGE OF ONLY 4 SHIFTS PER MONTH. WE JUST RECENTLY STARTED TO TRACK OUR ENGAGEMENTS DURING RELIEF OUTREACH. JUST LAST WEEK WE MADE MEANINGFUL CONTACT WITH 120 UNIQUE INDIVIDUALS. OUR HIGHEST LEVEL OF SERVICE, EMERGENCY SHELTER WITH INTENSIVE WRAP AROUND SERVICES, WAS DELIVERED TO 36 PEOPLE IN 2023. OF THOSE 36 PEOPLE, 24 ARE HOUSED AND ONE PASSED AWAY. WE ARE STILL WORKING WITH 5 OF THEM, ONE IS VERY CLOSE TO BEING ABLE TO RENT HER OWN APARTMENT.
