IN FY 2017, THE ORGANIZATION FOCUSED ON ASSISTING THE DISTRICT OF COLUMBIA'S DEPARTMENT OF HEALTH PREPARE A NUTRITION EDUCATOR GUIDE AND TRAINING TOOLKIT ON THE GUIDE. THIS ACTIVITY REQUIRED MEETING WITH OVER 40 INDIVIDUALS FROM ABOUT 25 ORGANIZATIONS THAT PROVIDE NUTRITION EDUCATION TO DC RESIDENTS. ABCD ALSO DEVELOPED A STRATEGIC PLAN, EVALUATION PACKAGE, AND MONTHLY COMMUNICATION PACKAGES FOR A CDC PROJECT, HEALTHY TIES THAT BIND, FOR THE LEADERSHIP COUNCIL FOR HEALTHY COMMUNITIES GRANT FROM CENTER FOR DISEASE CONTROL AND PREVENTION. THIS WORK FOR LCHC CONNECTS THE RESIDENTS OF WARDS 4, 5, 7, AND 8 WITH CLINICAL AND COMMUNITY PREVENTIVE HEALTH SERVICES TO SELF MANAGE THEIR DIABETES, HYPERTENSION, AND HEART DISEASE.
