HRSA PCA - PROVIDED NUMEROUS OPPORTUNITIES FOR COMMUNITY HEALTH CENTER LEADERS AND STAFF TO INTERACT AND LEARN FROM ONE ANOTHER AND FROM EXPERTS IN THE FIELD ABOUT IMPROVING THE DELIVERY OF HEALTH CARE TO PATIENTS. DRAMATICALLY INCREASED THE NUMBER OF HEALTH CENTERS BEING RECOGNIZED FOR PCMH AND QUALITY ACCOMPLISHMENTS. HCCN - THE ALASKA QUALITY IMPROVEMENT NETWORK (AQUIN), ALASKAS HEALTH CENTER CONTROLLED NETWORK (HCCN) IS A PROJECT FUNDED UNDER THE HEALTH RESOURCES AND SERVICES WITH A GOAL OF IMPROVING THE QUALITY OF CARE AND HEALTH OUTCOMES AT COMMUNITY HEALTH CENTERS. AQUIN IS COMPRISED OF 10 PARTICIPATING HEALTH CENTERS FROM ACROSS THE STATE. THE AQUIN MODEL UTILIZES A CENTRAL DATA WAREHOUSE, FED DAILY BY HEALTH CENTERS' ELECTRONIC HEALTH RECORDS (EHR). THE PLATFORM HAS A PORTFOLIO OF TOOLS AND PRE- FORMATTED REPORTS THAT ALLOW SOPHISTICATED ANALYTICS TO BE PERFORMED, RESULTING IN ACTIONABLE INFORMATION USED TO IMPROVE QUALITY. THE OVERALL PURPOSE OF THE AQUIN IS QUALITY IMPROVEMENT FOR BETTER HEALTH OUTCOMES, INCREASED PATIENT AND FAMILY ENGAGEMENT AND LOWER COSTS. THE DATA WAREHOUSE AIMS TO STRENGTHEN QUALITY, EFFICIENCY, AND SAFETY THROUGH THE DATA-DRIVEN SUPPORT OF PCMH MODELS, OPTIMIZATION OF THE CLINICS EHRS, AND ATTAINMENT OF MEANINGFUL USE. SCAHEC - FACILITATED 300+ INDIVIDUALS IN EXPLORING HEALTH CARE CAREERS. APPRENTICESHIP - THE ORGANIZATION IS A SUBAWARDEE OF THE STATE OF ALASKA DEPARTMENT OF LABOR AND WORKFORCE DEVELOPMENT (DOLWD), DIVISION OF EMPLOYMENT AND TRAINING SERVICES (DETS), UNDER THE "HEALTHY ALASKA THROUGH APPRENTICESHIP" GRANT FROM THE US DEPARTMENT OF LABOR.
