HOME CARE SERVICE (HCS):THE HOME CARE SERVICE (HCS) PROGRAM RECRUITS, SCREENS, TRAINS, AND EMPLOYS PERSONAL CARE ASSISTANTS (CAREGIVERS) TO PROVIDE SERVICES FOR QUALIFIED ELDERLY AND PHYSICALLY DISABLED PERSONS THROUGH THE ARIZONA LONG TERM CARE SYSTEM (ALTCS) THAT HAVE POLICIES UNDER UNITED HEALTHCARE COMMUNITY PLAN, MERCY CARE AND BANNER UNIVERSITY HEALTH PLAN. ABILITY360 ALSO PROVIDES HOME CARE SERVICES TO NATIVE AMERICANS LIVING OFF RESERVATION AND QUALIFY FOR SERVICES THROUGH ALTCS AND THE NATIVE AMERICAN COMMUNITY HEALTH CENTERS. IN ADDITION, ABILITY360 PROVIDES HOME CARE SERVICES TO PERSONS WHO ARE DEVELOPMENTALLY DISABLED AND QUALIFY FOR LONG-TERM CARE SERVICES UNDER THE ARIZONA DEPARTMENT OF ECONOMIC SECURITY'S DIVISION OF DEVELOPMENTAL DISABILITIES (DDD).PAS EMPLOYS OVER 1,500 PERSONAL CARE ASSISTANTS THROUGHOUT MARICOPA, PIMA, PINAL AND GILA COUNTIES.PERSONAL ASSISTANCE SERVICES WERE PROVIDED TO 1,500 CONSUMERS THROUGH THE PAS PROGRAM.
HOME MODIFICATION PROGRAMS (HMP):ABLITY360 INCREASES HOME ACCESSIBILITY, INDEPENDENCE, AND SAFETY FOR ALTCS MEMBERS LIVING WITH DISABILITIES. THE HOME MODIFICATION PROGRAM PROVIDES LICENSED CONTRACTORS TO PERFORM MODIFICATIONS SUCH AS WIDENING DOORWAYS, ENTRANCE RAMPS, INSTALLING GRAB BARS, ELEVATED TOILETS, AND OTHER NEEDED STRUCTURAL MODIFICATIONS.ABILITY360'S HOME MODIFICATION COMPLETED 3 ACCESSIBILITY PROJECTS THROUGH COMMUNITY DEVELOPMENT BLOCK GRANT (CDBG) CONTRACTS WITH SEVERAL VALLEY CITIES THAT INCLUDED PHOENIX, PEORIA, AND TEMPE. AN ADDITIONAL 96 HOME MODIFICATIONS WERE COMPLETED FOR CONSUMERS ELIGIBLE FOR THE ALTCS PROGRAM THROUGH HEALTHCARE PLAN MEMBERSHIPS WITH MERCY CARE FOR A TOTAL OF 99 HOME MODIFICATIONS. ABILITY360 REGULARLY PROVIDES PRESENTATIONS TO COMMUNITIES ON THE FAIR HOUSING ACT TO CONSUMERS, PROPERTY MANAGERS, AND CITY OFFICIALS. THIS EMPOWERS CONSUMERS TO UNDERSTAND THEIR RIGHTS WHICH MAKE A HUGE DIFFERENCE IN AVOIDING ISOLATION AND INSTITUTIONALIZATION.
TITLE VII PART C & TITLE VII PART C CARES ACT:THE SOCIALIZATION THROUGH RECREATION PROGRAM PROVIDED 51 PROGRAMS AND ACTIVITIES FOR 669 CONSUMERS INCLUDING 41 NEW CONSUMERS. VOLUNTEERS CONTRIBUTED 478 VOLUNTEER HOURS.THE REINTEGRATION FROM NURSING HOME TO COMMUNITY PROGRAM ASSISTED 15 CONSUMERS, 10 CONSUMERS IN LEAVING NURSING HOMES TO EXPERIENCE LESS RESTRICTIVE, MORE INDEPENDENT LIVING ARRANGEMENTS. A CORE SERVICE OF CILS.OUR EARLY INTERVENTION COORDINATOR CONTACTED 167 CONSUMERS THROUGH OUTREACH TO REHAB AND OTHER FACILITIES FOR THOSE WITH NEW INJURIES OR DISABILITIES AND ATTENDED 76 COMMUNITY OUTREACH MEETINGS, IMPACTING 966 PARTICIPANTS. INFORMATION & REFERRAL PROGRAM - 4,451 TOTAL CALLERS REQUESTING 5,654 TOPICAL QUESTIONS. 85 PRESENTATIONS WERE GIVEN TO 4,004 ATTENDEES. THIS IS ANOTHER CORE SERVICE OF CILS.INDEPENDENT LIVING SKILLS INSTRUCTION - 160 UNDUPLICATED CONSUMERS, 1,106 ONE-TO-ONE IL SKILLS TRAINING SESSIONS, ANOTHER CORE SERVICE OF CILS.EMPOWERING YOUTH - 39 INTERACTIVE WORKSHOPS WERE GIVEN TO STUDENTS WITH DISABILITIES AGES 14-21 ALL IN THE SCHOOL SETTINGS (VIRTUALLY), WITH A TOTAL OF 363 DUPLICATED COUNT, OR ROUGHLY 100 UNDUPLICATED STUDENTS. THIS IS ANOTHER CORE SERVICE OF CILS.PEER MENTOR MATCHES WERE MADE FOR 24 NEWLY INJURED CONSUMERS, AND 198 VOLUNTEER HOURS WERE CONTRIBUTED TO THIS PROGRAM.
