PERSONAL CARE SERVICES: SERVED AS FISCAL AGENT FOR MEDICAID MEMBERS USING PERSONAL CARE SERVICES WHICH PROVIDE RESOURCES TO PEOPLE WITH DISABILITIES TO HIRE PERSONAL CARE ATTENDANTS WHO ENABLE THEM TO LIVE WITHIN THE COMMUNITY. ALSO, IN A SEPARATE MEDICAID PROGRAM, HIRE AND MANAGE PERSONAL CARE ATTENDANTS FOR CONSUMERS WHO DO NOT WANT TO, OR ARE UNABLE TO, DIRECT THEIR OWN SERVICES.
INDEPENDENT LIVING: NURSING HOME TRANSITION AND YOUTH TRANSITION INTO THE COMMUNITY: PROVIDED SERVICES TO CONSUMERS TO ENCOURAGE AND EMPOWER PERSONS WITH DISABILITIES TO LIVE AS INDEPENDENTLY AS POSSIBLE. SERVICES PROVIDED INCLUDE INFORMATION AND REFERRAL, PEER COUNSELING, INDEPENDENT LIVING SKILLS TRAINING, AND INDIVIDUAL AND COMMUNITY ADVOCACY.
