THE MISSION OF TRUE CONNECTIONS COMMUNITY PROGRAMS IS TO HELP OUR CLIENTS BUILD LIFE SKILLS BASED ON CHOICES, INDIVIDUAL STRENGTHS, AND COMMUNITY INVOLVEMENT. WE CREATE AND NURTURE CONNECTIONS BETWEEN PEOPLE WITH DEVELOPMENTAL DISABILITIES AND THE DAVIS COMMUNITY THROUGH SOCIAL, COMMUNITY SERVICE AND PRE EMPLOYMENT SKILLS.

TRUE CONNECTIONS PROVIDED A VARIETY OF SERVICES TO THIRTY-FOUR ALTA REGIONAL CENTER CLIENTS DURING 2021. ALL OUR CLIENTS HAVE A DEVELOPMENTAL DISABILITY. BY DECEMBER OF 2021 WE WERE SERVING TWENTY-TWO CLIENTS IN PERSON AT OUR ADULT DAY PROGRAM FACILITY. THREE CLIENTS RECEIVED IN PERSON SERVICES THROUGH OUR TAILORED DAY PROGRAM. TAILORED DAY PROVIDES SUPPORT TO CLIENTS ENROLLED IN POST-SECONDARY SCHOOLS. (WOODLAND COMMUNITY COLLEGE, CSU SACRAMENTO, SACRAMENTO COMMUNITY COLLEGE). SEVEN OF OUR CLIENTS REMAINED AT HOME DUE TO THEIR COVID VACCINATION STATUS.COVID CONTINUED TO DAMPEN MANY OF OUR TRADITIONAL ACTIVITIES AT OUR DAY PROGRAM. OUR MAIN GOAL OF KEEPING CLIENTS AND STAFF SAFE DURING THE PANDEMIC WAS CHALLENGING. WE AVOIDED MANY OF OUR COMMUNITY OUTINGS AND REMAINED IN OUR FACILITY MORE THAN USUAL.TRUE CONNECTIONS FACILITY IS OPEN MONDAY THROUGH FRIDAY, FROM 9 AM TO 2PM. ALTA CALIFORNIA REGIONAL CENTER REFERS CLIENTS, THEY ALSO PAY THE DAILY FEE FOR EACH CLIENT.OUR FOCUS IS ON SOCIAL SKILLS, INDEPENDENT LIVING SKILLS, COMMUNITY SAFETY AND HEALTHY CHOICES. THE SERVICE WE PROVIDE INCLUDES DAILY ACTIVITIES TO CHOOSE TO DO ON SITE (CRAFTS, SCIENCE EXPERIMENTS, PUZZLES, SEWING, JEWELRY MAKING, IPAD ACTIVITIES, COMPUTER GAMES, DANCE PARTIES FOR EXERCISE). DAILY MEAL PREPARATION INCLUDING MENU PLANNING, SHOPPING, BUDGETING, SERVING AND CLEAN UP.COMMUNITY OUTINGS SUCH AS LEARNING TO RIDE THE LOCAL PUBLIC BUS SYSTEM AND HOW TO USE PHONE APPS TO DECIDE WHEN TO CATCH THE BUS. VOLUNTEERING AT DAVIS MEALS ON WHEELS. ANNUAL MEETINGS WITH REGIONAL CENTER STAFF TO DISCUSS CLIENT PROGRESS ON GOALS. WE ALSO PRODUCE ANNUAL REPORTS FOR EACH CLIENT, TO ENSURE TRUE CONNECTIONS MEETS THEIR NEEDS.
