SUPPORTED (COMMUNITY) LIVING PROGRAM- INDIVIDUALIZED TRAINING AND SUPPORT FOR ADULTS AND SENIORS ENABLING THEM TO LIVE IN THEIR OWN APARTMENTS OR HOMES. SERVICES RANGE FROM A FEW HOURS WEEKLY TO 24 HOUR SUPPORT. THE AMOUNT AND TYPE OF SUPPORT PROVIDED IS INDIVIDUALIZED ACCORDING TO THE PERSON'S NEEDS. SOME AREAS OF SUPPORT MAY INCLUDE DOMESTIC SKILLS, FINANCIAL MANAGEMENT, SOCIAL SKILLS, HEALTH AND HYGIENE, COMMUNITY INTEGRATION, TRANSPORTATION SKILLS, VOCATION AND PERSONAL GROWTH. TYPICALLY, INDIVIDUALS GO OUT TO WORK OR TO A DAY PROGRAM DURING THE DAY, WHICH MAY HAVE AN EDUCATIONAL OR VOCATIONAL FOCUS, OR IN SOME CASES, BOTH.
INTERMEDIATE CARE FACILITIES- THREE RESIDENTIAL FACILITIES PROVIDE COMPREHENSIVE SUPPORT SERVICES FOR 18 INDIVIDUALS WITH SEVERE PHYSICAL AND INTELLECTUAL DISABILITIES. STAFF ARE PRESENT ON A 24-HOUR, SEVEN DAYS A WEEK BASIS WITH REGISTERED NURSES AND DOCTORS FOR SUPPORT.
INDEPENDENT LIVING - TWO TRAINING HOMES PROVIDE AN EDUCATIONAL ENVIRONMENT FOR ADULTS, WITH MILD DEVELOPMENTAL DISABILITES, TO LEARN THE NECESSARY INDEPENDENT LIVING AND SOCIAL SKILLS REQUIRED FOR INDEPENDENT LIVING; AS WELL AS SOCIAL SKILLS REQUIRED FOR EFFECTIVE INDEPENDENT COMMUNITY LIVING.
