LIVING WELL YOUR WAY IS A SERVICE COORDINATION AGENCY SERVING 90 CONSUMERS IN 11 COUNTIES THROUGHOUT CENTRAL PENNSYLVANIA. WE HELP TO COORDINATE THE SERVICES AND CARE PROVIDED TO CONSUMERS RECEIVING HOME AND COMMUNITY BASED MEDICAID WAIVER SERVICES. OUR GOAL IS TO ASSIST INDIVIDUALS WITH PHYSICAL DISABILITIES IN REMAINING INDEPENDENT IN THE COMMUNITY AND IN THEIR HOMES SO THAT THEY DO NOT HAVE TO GO INTO NURSING FACILITIES. AS SERVICE COORDINATORS WE COORDINATE THE SERVICES AND MONITOR THE CARE PROVIDED TO PARTICIPANTS WE WORK WITH. WE ACT AS ADVOCATES, BECOMING A VOICE FOR INDIVIDUALS. WE HAVE ASSISTED INDIVIDUALS IN MODIFYING THEIR HOMES SO THAT THEY FEEL SAFER AND HAVE MORE ACCESS TO THEIR COMMUNITY. LWYW BOARD ELECTED TO CEASE OPERATIONS OF LWYW IN SEPTEMBER 2019.
