THE HEALTHY HOOSIERS FOUNDATION (HHF) IS A 501(C)(3) THAT WAS CREATED BY THE GENERAL ASSEMBLY IN 2013 (SEA 415). THE FOUNDATION WAS APPROVED TO SOLICIT AND ACCEPT PRIVATE FUNDING, GIFTS, DONATIONS, BEQUESTS, DEVISES, AND CONTRIBUTIONS. THE FOUNDATION PROVIDES FUNDING FOR PROGRAMS AT THE INDIANA DEPARTMENT OF HEALTH (DOH) THAT PROMOTES THE HEALTH AND WELL-BEING OF HOOSIERS. THE HHF IS DEDICATED TO A WIDE RANGE OF HEALTH ISSUES, BUT IN PARTICULAR THE HHF CONCENTRATES ON FUNDING PROGRAMS THAT: REDUCE INFANT MORTALITY, INCREASE CHILDHOOD IMMUNIZATIONS, REDUCE OBESITY, REDUCE SMOKING RATES.

FAMILIES FIRST: A COLLABORATIVE EVALUATION AND IMPROVEMENT OF THE FAMILY JOURNEY IN INDIANA. THE PURPOSE OF THIS GRANT IS: 1) TO EVALUATE THE CURRENT PROCESS FOR A HOOSIER FAMILY FROM IDENTIFICATION TO INTERVENTION; 2) BUILD CONSENSUS AROUND SYSTEMIC CHANGES TO BETTER INFORM AND SUPPORT.
