SERVICE/SUPPORTS COORDINATION IS A SERVICE THAT PROVIDES THE CONSUMER/PARTICIPANT WITH A SERVICE/SUPPORTS COORDINATOR, WHO WILL ASSIST THE CONSUMER/PARTICIPANT, WITH OBTAINING AND MAINTAINING SERVICES THAT ARE AVAILABLE IN THE HOME AND COMMUNITY SETTING. THESE SERVICES ALLOW THE CONSUMER/PARTICIPANT TO LIVE INDEPENDENTLY IN THEIR OWN HOME AND TO AVOID POTENTIAL PLACEMENT IN A NURSING FACILITY. A SERVICE/SUPPORTS COORDINATOR MEETS WITH CONSUMERS IN THEIR OWN HOME AND PROVIDES ADVOCACY AND SUPPORT.
