ALL PRIMARY CARE ASANTE PHYSICIAN PARTNERS CLINICS ARE CERTIFIED BY THE OREGON HEALTH AUTHORITY AS PATIENT-CENTERED PRIMARY CARE MEDICAL HOMES, MEETING THE REQUIRED STANDARDS OF HEALTH CARE COORDINATION THAT IMPROVE PATIENT OUTCOMES. HIGH-NEEDS PATIENTS SUCH AS THOSE WITH CHRONIC OR COMPLEX CONDITIONS ARE SUPPORTED BY INTERDISCIPLINARY TEAMS INCLUDING PHYSICIANS, NON-PHYSICIAN PROVIDERS, NURSES, MEDICAL ASSISTANTS, PHARMACISTS, BEHAVIORAL HEALTH CONSULTANTS, AS WELL AS OTHER HEALTH CARE PROFESSIONALS.IN ADDITION TO THE OHA CERTIFICATIONS EIGHT OF OUR PRIMARY CARE CLINICS PARTICIPATE IN MEDICARE'S COMPREHENSIVE PRIMARY CARE PLUS (CPC+) PROGRAM AND ONE OF OUR PRIMARY CARE CLINICS PARTICIPATE IN THE MEDICARE ADVANCED ALTERNATIVE PAYMENT PROGRAM ENTITLED PRIMARY CARE FIRST. THESE CMS PROGRAMS SEEK TO IMPROVE QUALITY AND ACCESS AND TO REDUCE HEALTHCARE EXPENSES THROUGH IMPROVING EFFICIENCY OF CARE. OUR PARTICIPATING PRACTICES CONTINUE TO MAKE STRIDES IN IMPROVING THE CARE THEY PROVIDE OUR COMMUNITY THROUGH FOCUS ON RISK STRATIFICATION, CARE MANAGEMENT, COMPREHENSIVE CARE COORDINATION BEYOND EPISODES OF CARE, PATIENT AND FAMILY ENGAGEMENT, AND TIMELY PATIENT FOLLOW UP AFTER EMERGENCY ROOM VISITS AND HOSPITALIZATIONS.THEIR LARGEST PROGRAM SERVICE ACCOMPLISHMENT TO TAKE PLACE DURING THE FISCAL YEAR ENDING SEPTEMBER 30, 2021 WAS TO SEE IN CLINIC 12,087 MEDICARE, 6,844 MEDICAID AND 1,269 UNINSURED PATIENTS. THESE PATIENTS MAY EXPERIENCE DIFFICULTY IN ACCESSING CARE IN OTHER PARTS OF THE COMMUNITY. ASANTE PHYSICIAN PARTNERS STANDS READY TO PROVIDE CARE FOR ALL THOSE IN NEED. THIS ACCESS IS OF PARTICULAR BENEFIT TO THE UNDERSERVED IN THE COMMUNITY.
MOST PHYSICIAN PRACTICES AND IN PARTICULAR SPECIALTY PRACTICES, ARE LOCATED WITHIN THE MORE HEAVILY POPULATED CITIES (MEDFORD, GRANTS PASS, ASHLAND AND JACKSONVILLE) IN THE PRIMARY SERVICE AREA. ASANTE EVALUATES AND PURSUES STRATEGIC ALLIANCES WITH PHYSICIANS AND OTHER PROVIDERS TO IMPROVE AVAILABILITY AND ACCESSIBILITY OF HEALTH CARE SERVICES IN OUR MARKET, LOWER COSTS AND ENHANCE THE QUALITY AND CONTINUITY OF HEALTH CARE SERVICES PROVIDED TO PATIENTS. THESE ALLIANCES CAN INCLUDE JOINT VENTURES, PARTNERSHIPS, SERVICE LINE AGREEMENTS, PHYSICIAN SERVICE AGREEMENTS, PARTICIPATION IN ASANTE HEALTH NETWORK OR OTHER RELATIONSHIPS. DURING FISCAL YEAR 2021, ASANTE PHYSICIAN PARTNERS RECRUITED 59 PROVIDERS FROM OUT OF THE AREA.
URGENT CARE CLINICS HAVE BEEN OPENED IN GRANTS PASS, MEDFORD AND WHITE CITY SINCE 2014 TO PROVIDE CONVENIENT AND AFFORDABLE ACCESS FOR QUICK DIAGNOSIS AND TREATMENT OF PATIENTS WITH NON-LIFE-THREATENING SYMPTOMS. TO FURHTER INCREASE ACCESS TO HEALTH CARE PROVIDERS, ASANTE BAGEN OFFERING URGENT CARE VIDEO VISITS IN 2019, SO PATIENTS CAN BE SEEN BY ASANTE PHYSICIAN PARTNERS PROVIDERS FROM THE COMFORT OF THEIR HOMES, ANYWHERE IN OREGON.WITH THE ONSET OF THE COVID 19 OUTBREAK, APP GREATLY EXPENADED ITS TELEMEDICINE CAPABILITIES AND FUNCTIONALITY FOR PATIENTS AND PROVIDERS. ALL PRIMARY CARE AND SPECIALTY CLINICS HAVE THE CAPABILITY TO OFFER TELEMEDICINE VISITS. TELEMEDICINE VISITS HAVE BEEN UTILIZED FOR SPECIALTY CONSULTS, POST HOSPITALIZATION VISITS, AND ROUTINE CARE. EXPANSION OF TELEMEDICINE HAS INCREASED PATIENT SATISFACTION AND EASE OF CARE.
