MOBILE MEALS AND WELLNESS CHECK PROGRAM - VOLUNTEERS PERFORM WELLNESS CHECKS AND DELIVER NUTRITIOUS HOT AND COLD MEALS PREPARED BY HONOR HEALTH THOMPSON PEAK HOSPITAL TO COMMUNITY MEMBERS ("FCC NEIGHBORS"), THOSE WHO ARE HOMEBOUND AND HAVE QUALIFYING NEEDS SUCH AS DISABLING CONDITIONS, SURGERY RECOVERY, ILLNESS, SUDDEN INJURY, MOBILITY CHALLENGES OR DIFFICULTY IN FOOD PREPARATION. ALL MEALS MEET AREA AGENCY ON AGING REGION ONE, INCORPORATED (PHOENIX, AZ) DIETARY GUIDELINES FOR THE AGING POPULATION AND ARE LOW IN SALT AND SUGAR TO HELP THOSE FOLLOWING A MODIFIED DIET. FINANCIALLY ELIGIBLE FCC NEIGHBORS HAVE THE COST OF THEIR MEALS PROVIDED BY A GRANT FROM THE AREA AGENCY ON AGING REGION ONE, INCORPORATED (PHOENIX, AZ). OTHER FCC NEIGHBORS PAY FOR THEIR MEALS AT A PRICE OF $7.25 PER MEAL. DURING FISCAL YEAR 2023/2024, THE FCC VOLUNTEERS FAITHFULLY CARRIED OUT ALL REQUIRED IN HOME WELLNESS CHECKS ENSURING OUR NEIGHBOR'S SAFETY IN THEIR HOMES AS THEY AGE IN PLACE AND SUCCESSFULLY DELIVERED APPROXIMATELY 19,342 NUTRITIOUS AND DELICIOUS MEALS.
TRANSPORTATION PROGRAMS - 1. NON-EMERGENCY MEDICAL TRANSPORTATION: VOLUNTEERS DRIVE THEIR OWN VEHICLES TO TRANSPORT AND PERSONALLY ACCOMPANY FCC NEIGHBORS TO AND/OR FROM DIALYSIS, CANCER TREATMENT, PHYSICAL THERAPY, AND OTHER MEDICAL AND DENTAL APPOINTMENTS MAKING SURE THAT EACH NEIGHBOR ARRIVES BACK HOME SAFE AND SOUND. 2. WHEEL SAFE: WITH ADDITIONAL PASS TRAINING, CERTIFIED FCC VOLUNTEERS DRIVE OR MAY SERVE AS AN ASSISTANT IN ONE OF THE 6 WHEELCHAIR ACCESSIBLE VANS TO SAFELY TRANSPORT AND TRANSFER FCC NEIGHBORS WHO HAVE SPECIAL WHEELCHAIR ACCESSIBLE TRANSPORTATION NEEDS TO AND FROM NON-EMERGENCY MEDICAL APPOINTMENTS. 3. RECREATIONAL VAN PROGRAM: FCC VOLUNTEERS DRIVE AND ACCOMPANY NEIGHBORS IN GROUPS TO SHOP AT GROCERY STORES AND LOCAL FOOD BANK, AS WELL AS TRANSPORT AND SUPPORT THEM AT SCHEDULED SOCIAL OUTINGS, INCLUDING FITNESS/EXERCISE CLASSES, EDUCATIONAL PROGRAMS, WELLNESS, AND A WIDE VARIETY OF COMMUNITY EVENTS TO HELP FIGHT SOCIAL ISOLATION AND PROMOTE A SENSE OF COMMUNITY. FCC OPERATES AND IS RESPONSIBLE FOR THE MAINTENANCE AND SAFETY OF A FLEET OF 9 VAN VEHICLES, 8 OF WHICH WERE ACQUIRED WITH PARTIAL FUNDING UNDER THE SECTION 5310 FEDERAL GRANT PROGRAM. SIX OF THE VEHICLES ARE WHEELCHAIR ACCESSIBLE. EACH VAN IS DRIVEN BY A VOLUNTEER DRIVER AND ACCOMPANIED BY VOLUNTEER ASSISTANTS AS NECESSARY. THE RECREATIONAL VAN TRIPS ARE POSTED MONTHLY IN THE SPECIAL FCC VAN CALENDAR MAILED DIGITALLY AND VIA USPS TO ALL FCC NEIGHBORS INTERESTED IN THE PROGRAM. DURING THE FYE 6/30/2024, 4,161 NON-EMERGENCY MEDICAL TRIPS AND 3,226 RECREATIONAL VAN TRIPS WERE PROVIDED. THOSE TRIPS TOTALED 94,833 TOTAL MILES DRIVEN.
INDIVIDUAL SERVICES - FCC VOLUNTEERS PROVIDE A VARIETY OF SERVICES TO FCC NEIGHBORS IN NEED INCLUDING: 1. HANDYMAN PROGRAM PROVIDES MINOR IN HOME REPAIRS; 2. FRIENDLY VISITING INCLUDES IN PERSON VISITS AND PHONE CALLS THAT PROVIDE COMPANIONSHIP, SUPPORT AND FRIENDSHIP FOR SOCIALLY ISOLATED NEIGHBORS, 3. CAREGIVER RELIEF-FCC VOLUNTEERS SPEND TIME WITH HOMEBOUND INDIVIDUALS WHILE THE PRIMARY CAREGIVER IS GIVEN THE TIME AND OPPORTUNITY TO REST, DO OTHER TASKS AND TAKE A BREAK; 4. FRIENDLY PET VISITING- VOLUNTEERS ACCOMPANY CERTIFIED PETS TO VISIT AND PROVIDE LOVE AND SUPPORT FOR SOCIALLY ISOLATED NEIGHBORS; 5. SHOPPING ASSISTANCE PROVIDES VOLUNTEERS TO SHOP WITH OR FOR NEIGHBORS; 6. TECHNOLOGY ASSISTANCE - VOLUNTEERS HELP NEIGHBORS WITH ISSUES INVOLVING THEIR COMPUTER OR SMART PHONE. A PROGRAM AVAILABLE NOT ONLY TO OUR NEIGHBORS, BUT ALSO TO THE GENERAL PUBLIC, IS OUR MOBILITY LOAN CLOSET- FCC LOANS, FREE OF CHARGE, ON A WALK-IN BASIS, A VARIETY OF MOBILITY MEDICAL EQUIPMENT DEVICES (WALKERS, WHEELCHAIRS, CANES, TRANSFER CHAIRS, CRUTCHES, SHOWER SEATS, COMMODES, ETC.).
