WE PROVIDE PROGRAMS AND SERVICES FOR OLDER ADULTS THAT RESIDE IN CAMERON, ELK AND MCKEAN COUNTIES IN PENNSYLVANIA. WE PERFORM PROTECTIVE SERVICES INVESTIGATIONS ON REPORTED ABUSE, NEGLECT, ABANDONMENT AND EXPLOITATION. WE MANAGE THE CARE OF OLDER ADULTS THROUGH THE OPTIONS PERSONAL CARE PROGRAM AND CAREGIVER SUPPORT PROGRAMS. WE OFFER CONGREGATE MEALS AND HEALTH/WELLNESS PROGRAMS IN 13 SENIOR CENTERS. WE PROVIDE FUNCTIONAL ELIGIBILITY DETERMINATION ASSESSMENTS, HOME DELIVERED MEALS, MEDICARE INSURANCE COUNSELING, INFORMATION/REFERRAL AND OMBUDSMAN SERVICES. WE ALSO PROVIDE MANAGEMENT OF DOMICILIARY CARE HOMES.

HUMAN SERVICES PROGRAM:OHS PURCHASED 364 DAILY VISITS IN A COMMUNITY ADULT DAY SERVICE PROGRAM FOR 13 OLDER ADULTS.OHS PERFORMED 903 FUNCTIONAL ELIGIBILITY DETERMINATION ASSESSMENTS FOR OLDER ADULTS IN NEED OF IN HOME CARE/NURSING FACILITY CARE SERVICES.OHS PURCHASED 32,999 CONGREGATE MEALS FOR 876 OLDER ADULTS PROVIDED AT 13 SENIOR CENTERS IN CAMERON, ELK AND MCKEAN COUNTIES. OHS REIMBURSED 336 CAREGIVERS AND OLDER ADULTS IN THE CAREGIVER SUPPORT PROGRAM AND OPTIONS CONSUMER REIMBURSEMENT PROGRAM.OHS PURCHASED 3,224 HOME DELIVERED MEALS FOR 43 OLDER ADULTSOHS PROVIDED 18,007 CALLERS WITH INFORMATION AND REFERRAL ASSISTANCE SERVICE.OHS PERFORMED 296 PROTECTIVE SERVICE INVESTIGATIONS FOR OLDER ADULTS ALLEGED TO BE VICTIMS OF ABUSE, NEGLECT OR EXPLOITATION.OHS RECEIVED AND RESPONDED TO 417 REPORTS OF NEED FOR THE PROTECTION OF ADULTS.OHS PROVIDED 772 OLDER ADULTS WITH SOCIAL, RECREATIONAL, EDUCATIONAL AND HEALTH AND WELLNESS PROGRAMS AT 13 SENIOR CENTERS IN CAMERON, ELK AND MCKEAN COUNTIES.


CARE MANAGEMENT: OHS PROVIDED CARE MANAGEMENT SERVICES TO OLDER ADULTS WHICH INCLUDED 559 ASSESSMENTS OF NEED THAT SUPPORT OLDER ADULTS STAYING IN THEIR HOME SAFELY AND INDEPENDENTLY.


PERSONAL CARE:OHS PURCHASED 63,562 HOURS OF PERSONAL CARE TO ASSIST 317 OLDER ADULTS.
