SOURCEPOINT'S "IN-HOME CARE PROGRAM" PROVIDES SERVICES DESIGNED TO HELP OLDER ADULTS LIVE SAFELY IN THEIR OWN HOMES WITH INDEPENDENCE AND DIGNITY. WE PROVIDE DIRECT ACCESS TO IN-HOME SERVICES, AS WELL AS REFERRALS TO COMMUNITY RESOURCES. IN-HOME SERVICES INCLUDE ADULT DAYCARE, CHORE SERVICES, EMERGENCY RESPONSE SYSTEMS, HOMEMAKER SERVICES, MEDICAL TRANSPORTATION, MENTAL HEALTH COUNSELING, NURSING SERVICES, PERSONAL CARE, AND RESPITE CARE.
SOURCEPOINT'S "COMMUNITY PROGRAMS" PROVIDE HUNDREDS OF ENGAGEMENT OPPORTUNITIES BOTH ON-SITE AT OUR ENRICHMENT CENTER, OFF-SITE THROUGHOUT THE COUNTY, AND ONLINE. PROGRAMS INCLUDE FITNESS, WELLNESS, ARTS, LEARNING, INSURANCE EDUCATION, FALLS PREVENTION, FAMILY CAREGIVER SUPPORT, AND MORE. IN ADDITION TO THE PROGRAMS WE DIRECTLY PROVIDE, SOURCEPOINT AWARDS COMMUNITY GRANTS EACH YEAR TO LOCAL ORGANIZATIONS THAT PROVIDE OTHER SERVICES TO OLDER ADULTS.
SOURCEPOINT'S "NUTRITION PROGRAM" PROVIDES OLDER ADULTS REGULAR ACCESS TO HEALTHY FOODS THROUGH MEALS ON WHEELS, COMMUNITY CAFES, AND FARMERS MARKET VOUCHERS FOR FRESH FOODS FROM PARTICIPATING MARKETS.
