ASSISTED OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES PREVIOUSLY INSTITUITIONALIZED WITH TRANSITION SUPPORT TO ALLOW THEM TO RETURN TO A COMMUNITY SETTING (THEIR HOME, HOME OF CAREGIVER, AFC, HOME FOR THE AGED).
