TO ADMINISTER SOCIAL WORK, NURSING AND OTHER HUMAN SERVICES UNDER THE MI CHOICE WAIVER PROGRAM FOR PERSONS 65 AND OLDER AND ADULTS WITH DISABILITIES. REGION 2 AAA SERVED 786 PARTICIPANTS IN OUR THREE COUNTY AREA. THE WAIVER PROGRAM IS INTENDED FOR SENIORS AND DISABLED INDIVIDUALS WHO REQUIRE A LEVEL OF CARE CONSISTENT WITH THAT WHICH IS PROVIDED IN A NURSING HOME, BUT WHO WISH TO REMAIN LIVING AT HOME OR IN THE COMMUNITY.
TO PROVIDE FUNDING AND OVERSIGHT FOR HOME DELIVERED MEAL PROGRAMS IN OUR THREE COUNTY AREA FOR ADULTS AGED 60 AND OVER, UNDER USDA TITLE III AND STATE FUNDING PROGRAMS. COLLECTIVELY, 477,000 MEALS WERE DELIVERED DURING THE FISCAL YEAR.
TO PROVIDE CARE MANAGEMENT SERVICES, INCLUDING ASSESSMENT, CARE PLAN DEVELOPMENT, AND COORDINATION OF IN-HOME AND OTHER SUPPORTIVE SERVICES FOR 324 INDIVIDUALS AGED 60 AND OVER.
OTHER PROGRAMS INCLUDE CONGREGATE MEALS, ELDER ABUSE PREVENTION, LEGAL AID, COMMUNITY EDUCATION, AND VARIOUS OTHER SERVICES TO IMPROVE THE LIVES OF SENIORS, DISABLED ADULTS AND THEIR CAREGIVERS.
