IS TO PROMOTE THE WELL-BEING OF OLDER ADULTS RESIDING IN FAYETTE, GREENE AND WASHINGTON COUNTIES THROUGH A PLANNED, COORDINATED, COLLABORATIVE APPROACH TO HEALTH AND HUMAN SERVICES TO ASSIST SENIORS WITH REMAINING IN THEIR HOMES AND COMMUNITIES FOR AS LONG AS POSSIBLE.
