NUTRITION PROGRAM--PROVIDE CONGREGATE AND HOME-DELIVERED NUTRITIONALLY BALANCED MEALS ON A DAILY BASIS TO ELIGIBLE ELDERLY AND DISABLED INDIVIDUALS IN MONTGOMERY, GREENE, MIAMI, DARKE, PREBLE, BUTLER, AND WARREN COUNTIES. TO BE ELIGIBLE, A PERSON IS 60 YEARS OR OLDER AND UNABLE TO PREPARE THEIR OWN MEALS ON A REGULAR BASIS, DUE TO BEING PHYSICALLY OR NEUROLOGICALLY IMPAIRED AND HAS NO ONE AVAILABLE TO PREPARE MEALS. THE PROGRAM ALSO ADDRESSES THE NUTRITIONAL NEEDS OF INDIVIDUALS UNDER 60 WHO ARE AFFLICTED WITH SOME TYPE OF DISABILITY.
ADULT DAY SERVICES--PROVIDE A SECURE ENVIRONMENT FOR INDIVIDUALS WHO ARE NO LONGER CAPABLE OF FULL-TIME INDEPENDENT LIVING AND REQUIRE SOME ASSISTANCE OR SUPERVISION. OUR TWO FACILITIES OFFER A COST EFFECTIVE ALTERNATIVE TO INSTITUTIONAL CARE WITH SERVICES THAT INCLUDE DAILY NURSING AND PERSONAL CARE, THERAPUTIC ACTIVITIES, NUTRITIONAL LUNCHES AND SNACKS, TRANSPORTATION TO AND FROM HOME BY CERTIFIED DRIVERS IN ADA APPROVED VEHICLES. OUR ADULT DAY FACILITIES ALLOW THE CAREGIVER AND FAMILY MEMBERS TO CONTINUE TO WORK WHILE GIVING THE PARTICIPANT AND FAMILY MEMBERS RESPITE, SUPPORT, AND EDUCATION.
HOME HEALTH CARE--PROVIDE PERSONAL CARE AND HOMEMAKING SERVICES TO ELDERLY AND DISABLED INDIVIDUALS RESIDING IN THE GREATER MIAMI VALLEY AREA. SERVICE PLANS AND ASSIST IN MAKING PROVISIONS FOR IN-HOME SERVICES SUCH AS HOMEMAKING, HOME HEALTH AID VISITS, MEALS AND FINANCIAL SERVICES. ALL OF THESE ACTIVITIES ARE AIMED AT PREVENTING INSTITUTIONALIZATION.
