CARE MANAGEMENT - CARE MANAGEMENT ACTIVITES OF AREA AGENCIES ON AGING SERVE AS A COORDINATIVE LINK BETWEEN THE IDENTIFICATION OF CONSUMER NEEDS AND THE TIMELY PROVISION OF APPROPRIATE SERVICES TO MEET THOSE NEEDS BY UTLIZING ALL AVAILABLE RESOURCES. ACTIVITIES INCLUDE, AT A MINIMUM, A COMPREHENSIVE NEEDS ASSESMENT OF THE CONSUMER; DEVELOPMENT OF A WRITTEN SERVICE PLAN; ARRANGEMENT, COORDINATION AND FOLLOW-UP OF SERVICE DELIVERY; AND ONGOING CASE RECORDING. DURING THE 2022 FISCAL YEAR, EXPERIENCE SERVICED 286 CONSUMERS IN THE CARE MANAGEMENT PROGRAM.
CONGREGATE MEALS - CONGREGATE MEALS ARE MEALS PROVIDED TO ELIGIBLE OLDER PERSONS IN A GROUP SETTING EITHER IN SENIOR CENTERS OR ADULT CARE CENTERS. HOT OR OTHER APPROPRIATE MEALS WHICH MEET AT LEAST ONE-THIRD OF THE RECOMMENDED NUTRITIONAL NEEDS OF OLDER PERSONS ARE AVAILABLE IN THE AREA SERVED BY THE AREA AGENCY ON AGING AT LEAST ONCE A DAY, FIVE OR MORE DAYS A WEEK. DURING THE 2022 FISCAL YEAR, EXPERIENCE SERVICED 184 CONSUMERS IN THE CONGREGATE MEALS PROGRAM.
HOME DELIVERED MEALS - THE HOME DELIVERED MEAL PROGRAM PROVIDES MEALS DELIVERED TO OLDER PENNSYLVANIANS WHO ARE UNABLE TO PREPARE THEIR OWN FOOD AND WHO CANNOT RELY ON FAMILY MEMBERS OR OTHER SUPPORTS TO SEE TO THEIR NUTRITIONAL NEEDS. AS PART OF THE HOME DELIVERED MEAL PROGRAM, AREA AGENCIES ON AGING PROVIDE APPROPRIATE INFORMATION ON NUTRITION AND GOOD EATING HABITS TO THE CONSUMER.
