Part 3 - Line 4a
ANEW PLACE PROVIDES A CONTINUUM OF CARE FOR HOMELESS ADULTS, ADDRESSING THE ROOT CAUSES OF HOMELESSNESS, HELPING INDIVIDUALS BUILD A STRONG FOUNDATION WITH THE TOOLS NEEDED TO START A NEW LIFE. FOUR PHASES PROVIDE EACH PERSON WITH A UNIQUE TRANSITION PLAN. DURING THE FIRST TWO PHASES, GUEST ARE HOUSED IN OUR SHELTER FACILITY. THE THIRD PHASE OFFERS TRANSITIONAL HOUSING. THE FINAL PHASE MOVES GRADUATES INTO INDEPENDENT LIVING AND WE CONTINUE TO PROVIDE AFTERCARE SUPPORT FOR TWO YEARS.
