PERSON CENTERED HOUSING OPTIONS (PCHO) IS AN INNOVATIVE NOT-FOR-PROFIT ORGANIZATION WHOSE MISSION IS TO ADDRESS, PREVENT AND ALLEVIATE CHRONIC HOMELESSNESS WITHIN MONROE COUNTY, NEW YORK AND THE SURROUNDING COMMUNITIES USING THE HOUSING FIRST MODEL.

HUD FUNDED PERMANENT SUPPORTIVE HOUSING PROGRAMS THAT ASSIST HOMELESS INDIVIDUALS AND/OR FAMILIES TO SECURE PERMANENT HOUSING AND PROVIDE SUPPORTIVE SERVICES ON-GOING CASES MANAGEMENT SERVICES TO FORMERLY HOMELESS INDIVIDUALS/FAMILIES ENROLLED IN PERMANENT SUPPORTIVE HOUSING PROGRAMS. THESE SERVICES WERE SUBSEQUENTLY PROVIDED BY PCHO DIRECTLY.
HOMELESS OUTREACH COVERS PCHO STAFF OUT ON THE STREETS OF ROCHESTER, NY, AND THE SURROUNDING MONROE COUNTY. THE OUTREACH TEAM WORKS TO CONNECT AND BUILD RELATIONSHIPS WITH HOMELESS INDIVIDUALS AND ULTIMATELY GET THEM PLACED INTO AGENDAS WHICH WILL ASSIST THEM IN OBTAINING PERMANENT HOUSING AND OTHER ASSISTANCE THROUGH COORDINATED ENTRY.
CARE MANAGEMENT INCLUDES PCHO'S HEALTH HOMES PROGRAM; A MEDICAID-FUNDED PROGRAM THAT PROVIDES CARE MANAGEMENT SERVICES TO ELIGIBLE MEDICAID ENROLLEES WITH THEIR CARE NEEDS SO THEY CAN STAY AS HEALTHY AS THEY CAN BE.
EMERGENCY SERVICES AND SHELTER PROVIDES SHORT-TERM HOUSING SOLUTIONS FOR FAMILIES IN CRISIS AND ASSIST IN TRANSITIONING THESE INDIVIDUALS TO STABLE LIVING SITUATIONS.
