CARE COORDINATION PROVIDES PROFESSIONAL GERIATRIC CARE MANAGERS WHO ASSESS SENIORS' NEED FOR SERVICES, DEVELOP CARE PLANS, AND MAINTAIN REGULAR CONTACT TO LENGTHEN THE TIME THE SENIOR CAN REMAIN AT HOME AND AVOID PLACEMENT IN A NURSING FACILITY. ALSO, THROUGH CARE COORDINATION, ELDERHELP'S POOL OF VOLUNTEERS OFFER REGULARLY SCHEDULED GROCERY SHOPPING, FRIENDLY VISITING, GARDENING AND HOME REPAIRS, ASSISTANCE WITH TRANSPORTATION TO MEDICAL APPOINTMENTS, AND OTHER SIMILAR ASSISTANCE TO SENIORS.
HOUSING SERVICES PROVIDES ASSISTANCE TO SENIORS IN NEED OF AFFORDABLE HOUSING OPTIONS INCLUDING RESIDENTIAL LIVING SOCIAL SERVICES AND ROOMMATE PLACEMENT SERVICES.
