OUR MISSION IS TO PROMOTE DIGNITY AND RESPECT FOR AGING, AND TO ADVOCATE FOR INDEPENDENCE IN THE LIVES OF PIMA COUNTY'S OLDER ADULTS AND THEIR FAMILIES, NOW AND FOR GENERATIONS TO COME.
Describe the Organization's Program Activity:
IN-HOME SERVICES:TO REMAIN LIVING SAFELY IN THEIR OWN HOMES, MANY OLDER ADULTS REQUIRE ASSISTANCE IN PERFORMING BASIC ACTIVITIES OF DAILY LIVING. PCOA'S COMMUNITY SERVICE SYSTEM PROVIDED 161,365 HOURS OF IN-HOME CARE FOR 1,689 OLDER ADULTS AND YOUNGER PERSONS WITH DISABILITIES WHO NEEDED ASSISTANCE WITH BATHING, LAUNDRY, CHANGING BED LINENS, CLEANING THEIR HOMES AND SHOPPING FOR GROCERIES.
MEAL PROGRAMS:PCOA IMPROVED THE NUTRITIONAL INTAKE OF 3,188 OLDER ADULTS THROUGH THE PROVISION OF 267,198 WELL-BALANCED PREPARED MEALS. THESE MEALS WERE SERVED AT 13 PIMA COUNTY SENIOR LUNCH SITES AND DELIVERED TO THE HOMES OF FRAIL OLDER ADULTS WHO COULD NO LONGER SAFELY COOK AND HAD DIFFICULTY LEAVING THEIR HOMES. 82% OF CONGREGATE AND 87% OF HOME-DELIVERED MEAL RECIPIENTS REPORTED THE MEAL WAS THEIR MAIN COOKED MEAL OF THE DAY.
PIMACARE AT HOME:PIMACARE AT HOME PROVIDES PERSONAL CARE, RESPITE CARE, AND HOMEMAKER SERVICES (LIGHT HOUSEKEEPING, LAUNDRY ASSISTANCE AND MEAL PREPARATION) FOR PIMA COUNTY RESIDENTS WHO ARE MEDICALLY ELIGIBLE FOR SERVICES UNDER ALTCS GUIDELINES. THE SERVICES ARE ALSO PROVIDED TO PRIVATE PAY CLIENTS. THE LEVEL OF CARE AND SERVICE IS BASED ON THE NEEDS AND DESIRES OF THE CLIENTS.
HOME REPAIR AND ADAPTATIONS CORRECTED HEALTH AND SAFETY HAZARDS, IMPROVED ACCESSIBILITY, MAINTAINED HEATERS AND COOLERS FOR 1063 LOW INCOME OLDER ADULTS WHO DID NOT HAVE SUFFICIENT INCOME TO MAKE THE REPAIRS AND NO FAMILY TO ASSIST.FAMILY CAREGIVER SUPPORT ENHANCED AND SUPPLEMENTED THE CARE PROVIDED BY FAMILY MEMBERS WHO WERE ASSISTING THEIR OLDER FAMILY MEMBERS THROUGH PCOA STAFF INFORMATION AND ASSISTANCE IN GAINING ACCESS TO SERVICES, SUPPORT GROUPS, TRAINING, AND TEMPORARY RELIEF FROM THE DEMANDS OF CARE GIVING THROUGH IN-HOME RESPITE CARE. 97% OF RESPITE CLIENTS REPORTED THE SERVICE MADE IT EASIER FOR THEM TO REMAIN IN THEIR HOME.THE MEDICARE INFORMATION & HEALTH INSURANCE ASSISTANCE PROGRAM ASSISTED 3,151 INDIVIDUALS IN MAKING INFORMED DECISIONS REGARDING MEDICARE PLANS INCLUDING PRESCRIPTION DRUG PLANS, RESOLVING CLAIMS, BILLING ISSUES AND REVIEWING EXISTING INSURANCE POLICY COVERAGE. PCOA ALSO PROVIDED CLASSES TO ASSIST NEW MEDICARE BENEFICIARIES IN MAKING INFORMED DECISIONS IN THE USE OF MEDICARE.THE PCOA HELPLINE PROVIDED INFORMATION AND REFERRAL TO 9,750 UNDUPLICATED INDIVIDUALS.THE LONG-TERM CARE OMBUDSMAN PROGRAM SERVED AS AN ADVOCATE FOR RESIDENTS OF NURSING AND ASSISTED LIVING CARE HOMES, ASSISTING THE RESIDENTS AND THEIR FAMILIES IN RESOLVING PROBLEMS AND CARE CONCERNS.HEALTH PROMOTION FOR OLDER ADULTS: 91% OF OLDER ADULTS HAVE AT LEAST ONE CHRONIC CONDITION; ONE IN THREE OLDER ADULTS FALL EACH YEAR. PCOA PROVIDED FOUR EVIDENCE-BASED PROGRAMS TO PROMOTE HEALTH AND WELLNESS; IMPROVE STRENGTH, FLEXIBILITY AND BALANCE; AND ADDRESS THE FEAR OF FALLING AND FALL RISKS. PCOA PROVIDED 19 "A MATTER OF BALANCE" CLASSES WITH 159 PARTICIPANTS, 12 ONGOING "ENHANCE FITNESS" CLASSES WITH 383 PARTICIPANTS, 1 "HEALTHY LIVING WITH DIABETES" CLASS WITH 12 PARTICIPANTS AND 1 "CHRONIC PAIN SELF-MANAGEMENT PROGRAM" CLASS WITH 13 PARTICIPANTS.PCOA PROMOTED VOLUNTEER AND COMMUNITY-BASED OUTREACH OPPORTUNITIES THROUGH NEIGHBORS CARE. THE NEIGHBORS CARE PROGRAM PROVIDED 120,574 VOLUNTEER HOURS, REACHING 2000 NEIGHBORHOOD RESIDENTS.
