STEPPINGSTONE SUPPORTS INDEPENDENT LIVING FOR ELDERS AND ADULTS WITH MEDICAL CHALLENGES THROUGH A DAY HEALTH PROGRAM THAT OPTIMIZES THE PHYSICAL AND PSYCHO-SOCIAL WELL-BEING OF SAN FRANCISCO'S CULTURALLY DIVERSE COMMUNITY.

ADULT DAY HEALTH CARE (ADHC) SERVICES ARE PART OF THE CONTINUUM OF COMMUNITY BASED SERVICES. THESE SERVICES ASSIST IN PREVENTING AND OFTEN TIMES, DELAYING INSTITUTIONAL PLACEMENT. ADHC SERVICES PROVIDE A VARIETY OF SUPPORT TO INDIVIDUALS AND THEIR FAMILIES, AS WELL AS RESPITE FOR CAREGIVERS. PARTICIPANTS MAY RECEIVE PHYSICAL, OCCUPATIONAL AND SPEECH THERAPIES, DIETARY AND NUTRITIONAL MONITORING, NURSING, PERSONAL CARE ASSISTANCE, TRANSPORTATION, A HOT MEAL, AND PSYCHIATRIC AND PSYCHOLOGICAL SERVICES, DEPENDING ON THEIR INDIVIDUAL NEEDS. STEPPINGSTONE PRIMARILY SERVES DISABLED ADULTS AND FRAIL SENIORS WITH PHYSICAL AND/OR MENTAL DISABILITIES. OUR ORGANIZATION SERVES APPROXIMATELY 280 INDIVIDUALS PER MONTH IN ONE OF OUR FOUR CENTERS.
