YOUTH AND FAMILY SERVICES PROGRAM:YOUTH AND FAMILY SERVICES PROVIDES A CONTINUUM OF SUPPORT TO YOUTH WITH DISABILITIES, THEIR FAMILIES, AND UNACCOMPANIED YOUTH, ASSISTING THEM TO IDENTIFY GOALS AND CREATE A PLAN OF HOW TO ACHIEVE THEM. SUPPORT ACTIVITIES ACROSS PROGRAMS INCLUDE ASSESSING NEEDS AND STRENGTHS, IDENTIFYING DESIRED OUTCOMES, MAKING REFERRALS, ATTENDING RELATED MEETINGS WITH CLIENTS, FACILITATING A TEAM-BASED PROCESS AND PLAN OF CARE, ENGAGING COLLABORATIVELY WITH OTHER SERVICE PROVIDERS, ASSISTING CLIENTS WITH APPLICATIONS FOR SERVICES, STABILIZING EDUCATION PLANS, AND ENGAGING NATURAL SUPPORTS. PROGRAMS INCLUDE SERVICE COORDINATION, HEALTHY TRANSITIONS, SKILL BUILDING, AND WRAPAROUND.A TOTAL OF 472 YOUTH WITH SPECIAL NEEDS AND DISABILITIES SERVED ACROSS PROGRAMS.
