YOUTH AND FAMILY SERVICES PROGRAM:YOUTH AND FAMILY SERVICES PROVIDES A CONTINUUM OF SUPPORT TO YOUTH WITH DISABILITIES, THEIR FAMILIES, AND UNACCOMPANIED YOUTH, ASSISTING THEM TO IDENTIFY GOALS AND CREATE A PLAN OF HOW TO ACHIEVE THEM. SUPPORT ACTIVITIES ACROSS PROGRAMS INCLUDE ASSESSING NEEDS AND STRENGTHS, IDENTIFYING DESIRED OUTCOMES, MAKING REFERRALS, ATTENDING RELATED MEETINGS WITH CLIENTS, FACILITATING A TEAM-BASED PROCESS AND PLAN OF CARE, ENGAGING COLLABORATIVELY WITH OTHER SERVICE PROVIDERS, ASSISTING CLIENTS WITH APPLICATIONS FOR SERVICES, STABILIZING EDUCATION PLANS, AND ENGAGING NATURAL SUPPORTS.
