PACE SOUTHEAST MICHIGAN (PACE) IS AN ALTERNATIVE TO NURSING HOME CARE FOR THE FRAIL AND ELDERLY. THE GOAL IS TO HELP INDIVIDUALS REMAIN INDEPENDENT IN THEIR OWN HOMES AND COMMUNITIES AND TO PROVIDE RESPITE AND ASSISTANCE TO FAMILY CAREGIVERS. PACE IS A PROGRAM WHICH PROVIDES COMPREHENSIVE MEDICAL AND SUPPORTIVE SERVICES. THE INTERDISCIPLINARY TEAM AT PACE ASSESSES POTENTIAL PARTICIPANTS AND DEVELOPS A PERSON-CENTERED CARE PLAN - CUSTOMIZING SERVICES TO MEET INDIVIDUALS' NEEDS. FREQUENT CONTACTS WITH PARTICIPANTS AND REGULAR REVIEWS OF THE CARE PLAN ENSURE THAT PARTICIPANTS' NEEDS ARE BEING MET.
