MERCY HOME HEALTH PROVIDES A RANGE OF HOME-BASED HEALTH CARE SERVICES THROUGHOUT THE FIVE-COUNTY PHILADELPHIA AREA. SERVICES PROVIDED INCLUDE SKILLED CARE NEEDED AFTER HOSPITALIZATION, REHABILITATIVE CARE AFTER ORTHOPEDIC JOINT REPLACEMENT, SHORT-TERM CARE FOLLOWING A WORK RELATED INJURY, AND EDUCATION TO HELP MANAGE A CHRONIC ILLNESS. MERCY HOME HEALTH NURSES AND THERAPISTS ASSESS EACH PATIENT'S MEDICAL CONDITION, HOME ENVIRONMENT AND SUPPORT SYSTEM TO DETERMINE A CUSTOM CARE PLAN.
