SERVICES ARE PROVIDED WHEREVER THE PATIENT CALLS 'HOME': IN A RESIDENTIAL HOME, LONG TERM CARE FACILITIES, AND OUR LICENSED HOSPICE INPATIENT FACILITY. WE SERVE EL PASO AND TELLER COUNTIES AND SURROUNDING COMMUNITIES. PIKES PEAK HOSPICE AND PALLIATIVE CARE PROVIDES ADVANCED ILLNESS MANAGEMENT FOR INDIVIDUALS LIVING WITH SERIOUS ILLNESS. WE PROVIDE PAIN AND SYMPTOM MANAGEMENT THROUGH COLLABORATION WITH THE PATIENT'S PRIMARY CARE PHYSICIAN THROUGH OUR NURSE PRACTITIONERS, NURSES AND SOCIAL WORK SERVICES.
