HOME CARE, HOSPICE AND BEREAVEMENT CARE, AND PRIVATE DUTY CARE PROVIDED TO RESIDENTS OF WESTERN CENTRAL COLORADO. APPROXIMATELY 11,976 HOSPICE DAYS, 8,964 HOME HEALTH VISITS, AND 8,549 PRIVATE DUTY HOURS WERE PROVIDED IN 2021. AS THE LARGEST HOME CARE AND HOSPICE PROVIDER IN CENTRAL WESTERN COLORADO, WE PROVIDE HOME AND HOSPICE CARE TO PATIENTS IN OVER 6,000 SQUARE MILES. THOUGH THE LARGEST PERCENTAGE OF OUR PATIENTS ARE MEDICARE, WE ALSO SERVE MEDICAID/VA PATIENTS AND PRIVATE PAY PATIENTS.
