HOSPICE OF THE PRAIRIE, INC. IS A NOT-FOR-PROFIT, MULTI-DISCIPLINARY ORGANIZATION OFFERING HOSPICE AND OTHER HOME CARE SERVICES IN A SUPPORTIVE AND COMPASSIONATE MANNER. DURING 2021, THE AVERAGE DAILY CENSUS OF HOSPICE PATIENTS WAS 29.68 PATIENTS WITH A MEDIAN LENGTH OF STAY OF 13.42 DAYS. 215 PATIENTS WERE ADMITTED TO THE PROGRAM DURING THE YEAR. DURING 2021, THE AVERAGE DAILY CENSUS OF HOME HEALTH PATIENTS WAS 56.58 AND 421 PATIENTS WERE ADMITTED TO THE PROGRAM.

HOSPICE OF THE PRAIRIE DIVISIONTHE MAIN PURPOSE OF THIS DIVISION IS TO PROVIDE MEDICAL CARE AND SUPPORTIVE SERVICES TO TERMINALLY ILL PATIENTS AND THEIR FAMILIES. THESE SERVICES MAY BE PROVIDED IN NURSING HOMES, ASSISTED LIVING FACILITIES OR IN THE PATIENT'S OWN HOME. HOSPICE SERVICES ARE PROVIDED THROUGH AN INTERDISCIPLINARY TEAM APPROACH, DIRECTED BY THE PATIENT'S OWN PHYSICIAN. HOSPICE SERVICES ALLOW PERSONS TO FACE THE REALITY OF A LIFE-THREATENING ILLNESS WITH HOPE AND DIGNITY, SURROUNDED BY FAMILY AND FRIENDS. PHYSICAL, PSYCHOLOGICAL, SOCIAL AND SPIRITUAL CARE ARE AVAILABLE FROM THE MEDICALLY DIRECTED TEAM CONSISTING OF: PHYSICIAN/NURSING CARE, HOME HEALTH AIDE, SOCIAL WORK, PASTORAL CARE, VOLUNTEERS AND BEREAVEMENT.
PRAIRIE HOME HEALTH DIVISIONTHE MAIN PURPOSE OF THIS DIVISION IS TO PROVIDE MEDICAL CARE AND SUPPORTIVE SERVICES TO RECOVERING, DISABLED OR CHRONICALLY ILL PERSONS WHO NEED MEDICAL TREATMENT OR ASSISTANCE WITH DAILY LIVING. THROUGH THE HOME HEALTH DIVISION, CLIENTS RECEIVE QUALITY SKILLED SERVICES IN THE COMFORT AND SECURITY OF THEIR OWN HOMES. SERVICES AVAILABLE INCLUDE NURSING CARE, HOME HEALTH AIDE AND RESTORATIVE THERAPY (PHYSICAL, OCCUPATIONAL, SPEECH). ADDITIONALLY, MEDICAL SOCIAL SERVICES ARE PROVIDED FOR SHORT TERM THERAPEUTIC COUNSELING AND TO ASSIST WITH REFERRALS TO OTHER COMMUNITY AGENCIES.
