PROVIDED MEDICAL CARE FOR 453 PATIENTS AND 36,935 HOME CARE AND INPATIENT DAYS. PROVIDED VARIOUS COMMUNITY SERVICES, SUCH AS BEREAVEMENT COUNSELING AND CAREGIVER SEMINARS AND CLASSES. A LENDING LIBRARY IS AVAILABLE TO THE COMMUNITY AND NUMEROUS CLASSES ON VARIOUS MEDICAL RELATED ISSUES ARE OFFERED. ATTEND VARIOUS COMMUNITY HEALTH FAIRS AND PROVIDE GRIEF, CRISIS, AND SPECIALIZED COUNSELING TO MEMBERS OF THE COMMUNITY. VARIOUS EMPLOYEES ARE AVAILABLE AT ALL TIMES FOR PUBLIC SPEAKING ON HOSPICE AND BEREAVEMENT RELATED ISSUES. THE TRANSITIONS PROGRAM IS INTENDED TO PROVIDE NO-COST ASSISTANCE TO THOSE IN THE COMMUNITY FACING LIFE-LIMITING ILLNESS, BUT DO NOT MEET MEDICARE HOSPICE STANDARDS. THIS NON-MEDICAL PROGRAM PROVIDES SUPPORT AND INFORMATION TO PARTICIPANTS AND THEIR FAMILIES.
