MIDLAND CARE SENIOR SERVICES INCLUDE THE PROVISION OF A PROGRAM FOR ALL INCLUSIVE CARE FOR THE ELDERLY (PACE) PROGRAM, FOR A MULTI-COUNTY AREA. THIS PROGRAM COORDINATES ALL MEDICAL AND HOMECARE SERVICES FOR INDIVIDUALS WITH MULTIPLE CHRONIC CONDITIONS OR COMPLEX CARE NEEDS TO ASSIST THEM TO STAY IN THE COMMUNITY, AVOID HOSPITALIZATIONS AND IMPROVE OVERALL QUALITY OF LIFE THROUGH PATIENT CENTERED SERVICES. ALSO, INCLUDES ADULT DAY CARE, MEDICAL TRANSPORTATION, RESPITE SERVICES AND SOME SENIOR HOUSING.
MIDLAND CARE CONNECTION, INC IS A NOT-FOR-PROFIT COMMITTED TO HELPING PEOPLE WITH HEALTH CARE CHALLENGES STAY IN THEIR HOMES WHENEVER POSSIBLE. MIDLAND CARE HAS DEVELOPED A FULL CONTINUUM OF COMMUNITY BASED SERVICES TO SUPPORT PATIENTS WITH NON-MEDICAL SUPPORTIVE CARE NEEDS ALL THE WAY THROUGH SKILLED CARE NEEDS AND TO END-OF-LIFE AND BEREAVEMENT SUPPORT TO SURVIVING FAMILY MEMBERS. OUR HOSPICE PROGRAM PROVIDES CARE TO ALL AGES AND ALL DIAGNOSES AND INCLUDES BOTH HOME CARE AS WELL AS THE AVAILABILITY OF OUR HOSPICE HOUSE FOR INPATIENT STAYS FOR COMPLEX PAIN AND SYMPTOM MANAGEMENT AND RESPITE CARE.
