COMING HOME CONNECTION'S MISSION IS TO LEAD, COALESCE, AND ACTIVATE A COMMUNITY OF SUPPORT FOR OLDER ADULTS AND THEIR FAMILIES THAT PROVIDES TRUSTED, HIGH QUALITY AND COMPASSIONATE IN-HOME CAREGIVING, RESOURCES, AND END OF LIFE CARE FOR ALL IN NEED, NO MATTER THEIR ABILITY TO PAY.

IN-HOME CARE IN 2023, COMING HOME CONNECTION PROVIDED IN-HOME CAREGIVING SERVICES TO 91 CLIENTS. SERVICES INCLUDE POST-SURGERY SUPPORT, COMPANIONSHIP AND LONG- TERM CARE ALIGNED TO INDIVIDUAL CARE NEEDS.


NO-COST CAREGIVING & HOUSEKEEPING - "TAKING CARE OF NEIGHBORS" THE "TAKING CARE OF NEIGHBORS" PROGRAM PROVIDED NO-COST SERVICES FOR 32 INDIVIDUALS WITHOUT THE ABILITY TO PAY FOR IN-HOME CAREGIVING AND HOUSEKEEPING. VOUNTEER COMPANIONS AN ADDITIONAL 10 NEW OLDER ADULTS WERE CONNECTED TO VOLUNTEERS FOR SUPPORT AND COMPANIONSHIP IN 2023. IN TOTAL, 23 OLDER ADULTS RECEIVED VOLUNTEER SERVICES TO COMBAT ISOLATION AND HELP THEM AGE IN THE PLACE OF THEIR CHOICE.


HEALTH NAVIGATION THE COMING HOME CONNECTION HEALTH NAVIGATOR WORKED WITH 61 CLIENTS, CREATING NAVIGATION PLANS TO HELP THEM ACCESS MEDICAID AND MEDICAID WAIVER SERVICES. IN TOTAL 157 RESIDENTS RECEIVED SUPPORT AND REFERRALS TO PUBLIC RESOURCES, LOCAL FOOD BANKS, UTILITY ASSISTANCE AND COMMUNITY-BASED RESOURCES LIKE MENTAL HEALTH CARE AND HOUSING OPTIONS.


EQUIPMENT LENDING & DONATING THE DURABLE EQUIPMENT LOAN PROGRAM PROVIDED OVER 1,500 PIECES OF EQUIPMENT, SUPPORTING MORE THAN 700 FAMILIES WITH NO-COST LOANS OF EQUIPMENT SUCH AS WHEELCHAIRS, WALKERS, ROLLATORS AND BEDSIDE COMMODES.
