IN-HOME CARE IN 2021, COMING HOME CONNECTION PROVIDED IN-HOME CAREGIVING TO 59 CLIENTS WITH 4,800 VISITS (TOTALING 37,000 HOURS) BY 12 VOLUNTEERS AND 47 PROFESSIONAL CAREGIVERS. THE HEALTH NAVIGATION PROGRAM SERVED 41 CLIENTS WITH CASE-MANAGEMENT AND REFERRALS TO RESOURCES TO MEET CLIENT NEEDS.
CASA CIELO CASA CIELO, A HOSPICE FACILITY, ASSISTED A NUMBER OF SANTA FE RESIDENTS FOR WHOM DYING AT HOME WAS NOT AN OPTION WITH END-OF-LIFE CARE. THE FACILITY WAS CLOSED IN SEPTEMBER 2021.
EQUIPMENT LENDING & DONATING THE DURABLE EQUIPMENT LOAN PROGRAM PROVIDED 320 PIECES OF EQUIPMENT TO 212 FAMILIES.
