COMING HOME CONNECTION TRAINS, PLACES AND SUPPORTS VOLUNTEER AND PROFESSIONAL CAREGIVERS IN HOMES AND OTHER SETTINGS WHERE HELP IS NEEDED, TO ASSIST CLIENTS AND THEIR FAMILIES THROUGH SICKNESS, OLD AGE AND THE END OF LIFE.

IN-HOME CARE IN 2021, COMING HOME CONNECTION PROVIDED IN-HOME CAREGIVING TO 59 CLIENTS WITH 4,800 VISITS (TOTALING 37,000 HOURS) BY 12 VOLUNTEERS AND 47 PROFESSIONAL CAREGIVERS. THE HEALTH NAVIGATION PROGRAM SERVED 41 CLIENTS WITH CASE-MANAGEMENT AND REFERRALS TO RESOURCES TO MEET CLIENT NEEDS.


CASA CIELO CASA CIELO, A HOSPICE FACILITY, ASSISTED A NUMBER OF SANTA FE RESIDENTS FOR WHOM DYING AT HOME WAS NOT AN OPTION WITH END-OF-LIFE CARE. THE FACILITY WAS CLOSED IN SEPTEMBER 2021.


EQUIPMENT LENDING & DONATING THE DURABLE EQUIPMENT LOAN PROGRAM PROVIDED 320 PIECES OF EQUIPMENT TO 212 FAMILIES.
