CRHS LONG TERM AND HOME CARE, INC. IS A NOT-FOR-PROFIT ORGANIZATION. ON MARCH 1, 2012, CRHS LONG TERM AND HOME CARE, INC. ENTERED INTO AN ASSET PURCHASE AGREEMENT WITH THE HOSPITAL AUTHORITY OF COLUMBUS TO SELL ALL THE ASSETS OF AZALEA TRACE NURSING HOME TO THE AUTHORITY. AT THAT TIME, MUSCOGEE MANOR (OPERATED BY THE HOSPITAL AUTHORITY OF COLUMBUS) ASSUMED OWNERSHIP AND FULL OPERATION OF AZALEA TRACE NURSING HOME. CRHS LONG TERM AND HOME CARE IS LEASING THE BUILDING TO THE HOSPITAL AUTHORITY UNTIL THE AUTHORITY IS ABLE TO FINISH CONSTRUCTION OF A NEW FACILITY.
