THE MISSION OF FAMILY HEALTH CENTERS OF SOUTHWEST FLORIDA, INC IS TO PROVIDE HIGH QUALITY HEALTH CARE SERVICES ACCESSIBLE TO ALL PEOPLE, BUT SIGNIFICANTLY SERVICING LOWER INCOME PATIENTS. FAMILY HEALTH CENTERS IS A MEDICAID PROVIDER AND A FQHC.

THE ORGANIZATION PROVIDED QUALITY MEDICAL, DENTAL, AND ANCILLARY SERVICES TO PATIENTS AS FOLLOWS: MEDICAL SERVICES WERE PROVIDED TO 75,240 PATIENTS WITH TOTAL OF 205,857 PATIENT VISITS. DENTAL SERVICES WERE PROVIDED TO 36,493 PATIENTS WITH A TOTAL OF 112,265 PATIENT VISITS. MENTAL HEALTH SERVICES WERE PROVIDED TO 433 PATIENTS WITH A TOTAL OF 1,092 PATIENT VISITS. OPTOMETRY SERVICES WERE PROVIDED TO 5,143 PATIENTS WITH A TOTAL OF 5,690 PATIENT VISITS. PODIATRY SERVICES WERE PROVIDED TO 2,199 PATIENTS WITH A TOTAL OF 3,696 PATIENT VISITS. PATIENTS WERE SEEN REGARDLESS OF THEIR ABILITY TO PAY. A DISCOUNTED FEE SCHEDULE IS USED, BASED ON THE FEDERAL POVERTY GUIDELINES, TO DETERMINE THE AMOUNT DUE FROM QUALIFYING PATIENTS.
