VISITING NURSE HEALTH SYSTEM PROVIDES HOME HEALTHCARE SERVICES TO OVER 4,600 PATIENTS ANNUALLY IN THEIR HOMES BY LICENSED, PROFESSIONAL HOME HEALTHCARE CAREGIVERS. SERVICES ENCOMPASS A BROAD RANGE OF CARE AND SUPPORT SERVICES FOR PATIENTS RECOVERING FROM A HOSPITAL STAY, WHO MAY BE CHRONICALLY ILL, OR NEED ASSISTANCE WITH THE ESSENTIAL ACTIVITIES OF DAILY LIVING. THIS CARE IS PROVIDED BY A TEAM OF APPROXIMATELY 100 PROFESSIONAL, SKILLED MEDICAL PERSONNEL, INCLUDING NURSES, PHYSICAL AND OCCUPATIONAL THERAPISTS, HOME HEALTHCARE AIDES AND SOCIAL WORKERS. THE PRIMARY GOALS OF HOME HEALTHCARE ARE TO HELP PATIENTS AVOID HOSPITAL READMISSION, IMPROVE THEIR CLINICAL OUTCOMES, AND IMPROVE THEIR OVERALL HEALTH. AS A LEADER IN THE FIELD, VISITING NURSE HEALTH SYSTEM DELIVERS QUALITY, COST-EFFECTIVE CARE WITH THE UTMOST COMPASSION, AND REMAINS STEADFAST IN THEIR COMMITTMENT TO HELPING PATIENTS REMAIN AS INDEPENDENT AS POSSIBLE. EQUALLY AS IMPORTANT, VISITING NURSE CONTINUES TO STRENGTHEN THEIR COMMITMENT TO THE COMMUNITIES IT SERVES, AND TO PROVIDING QUALITY CARE TO ALL WHO NEED IT USING THE RESOURCES AVAILABLE.
THE PRIMARY GOAL OF VISITING NURSE HEALTH SYSTEM'S COMMUNITY CARE DIVISION IS TO PROVIDE QUALITY, IN-HOME HEALTHCARE, HELPING PATIENTS REMAIN LIVING INDEPENDENTLY AS LONG AS POSSIBLE. ALONG WITH THE COMMUNITY CARE STAFF, THIS DIVISION OF VISITING NURSE PARTNERS WITH A BROAD RANGE OF COMMUNITY ORGANIZATIONS TO PROVIDE A COMPREHENSIVE RANGE OF SERVICES TO NURSING HOME-ELIGIBLE, LOW-INCOME SENIORS AND DISABLED INDIVIDUALS WHO HAVE ONGOING MEDICAL CONDITIONS REQUIRING LONG-TERM SUPPORT. THESE COMMUNITY CARE PROGRAMS INCLUDE SERVICE OPTIONS USING RESOURCES IN A COMMUNITY ENVIRONMENT (SOURCE), COMMUNITY CARE SERVICES PROGRAM (CCSP) AND HOME AND COMMUNITY BASED SERVICES (HCBS). A TEAM OF APPROXIMATELY 100 SOCIAL WORKERS AND NURSES CAREFULLY DETERMINE EACH PATIENT'S INDIVIDUAL, LONG-TERM CARE NEEDS, AND COORDINATE ACCESS TO A VARIETY OF LOCAL MEDICAL AND SOCIAL SERVICE RESOURCES FOR NEARLY 6,000 SENIORS AND THEIR FAMILY MEMBERS EACH YEAR.
VISITING NURSE HEALTH SYSTEM| HOSPICE ATLANTA PROVIDES HOSPICE AND PALLIATIVE CARE SERVICES TO PATIENTS WITH LIFE-LIMITING ILLNESSES WHO HAVE ELECTED TO STOP AGGRESSIVE, CURATIVE TREATMENTS. THE PRIMARY GOALS OF HOSPICE ATLANTA ARE TO PROVIDE SAFETY, COMFORT AND DIGNITY TO OUR PATIENTS DURING THE FINAL STAGE OF LIFE, PROVIDE THEIR FAMILIES WITH THE NECESSARY SERVICES AND EMOTIONAL SUPPORT DURING THE DYING PROCESS AND BEREAVEMENT SUPPORT AFTER THE DEATH OF THEIR LOVED ONE. OUR TEAM OF APPROXIMATELY 45 PROFESSIONALS AND 85 SPECIALLY TRAINED VOLUNTEERS PROVIDE END-OF-LIFE CARE TO NEARLY 500 PATIENTS ANNUALLY. HOSPICE ATLANTA PROVIDES COMPASSIONATE HOSPICE CARE FOR PATIENTS IN THEIR OWN HOMES, WHERE THEY PREFER TO BE. HOSPICE ATLANTA OFFERS A WIDE RANGE OF EMOTIONAL AND SPIRITUAL BEREAVEMENT SUPPORT SERVICES, PROVIDED BY TRAINED BEREAVEMENT COORDINATORS AND BEREAVEMENT PROFESSIONALS. IN ADDITION, HOSPICE ATLANTA'S SUPPORT INCLUDES IN-PERSON COUNSELING OR OVER-THE-PHONE SUPPORT, REGULARLY SCHEDULED GRIEF SUPPORT GROUPS, EDUCATIONAL MATERIALS AND THE TWICE-YEARLY CAMP S.T.A.R.S. BEREAVEMENT CAMP. BEREAVEMENT SUPPORT IS AVAILABLE FOR ONE YEAR AFTER THE DEATH OF A LOVED ONE, TWO YEARS FOR THOSE MOURNING THE LOSS OF A CHILD.
