THIS DETAILED PROCESS RESULTING IN AN ADVANCED DIRECTIVE HELPS INDIVIDUALS IDENTIFY AND CLARIFY THEIR PERSONAL VALUES AND GOALS OF CARE.HONORING CHOICES FLORIDA IS BASED UPON AN INTERNATIONALLY RECOGNIZED EVIDENCE-BASED ADVANCE CARE PLANNING MODEL, RESPECTING CHOICES, AND WAS LAUNCHED BY COMMUNITY HOSPICE & PALLIATIVE CARE IN PARTNERSHIP WITH HEALTH SYSTEM AND COMMUNITY PROVIDERS.
