HOME CARE - THE AGENCY'S GOAL IS TO HELP CLIENTS MAINTAIN THEIR INDEPENDENCE BY REMAINING IN THEIR OWN HOMES FOR AS LONG AS IS FEASIBLY POSSIBLE. HOME CARE SERVICES INCLUDE ASSISTANCE WITH THE ACTIVITIES OF DAILY LIVING, COMPANIONSHIP, TRANSPORTATION AND HOME-MAKING AS WELL AS NURSING ASSESSMENTS TO DEVELOP CARE PLANS. THIS PROGRAM SERVED 1,543 CLIENTS DURING THE FISCAL YEAR ENDED MAY 31, 2022.
OUTPATIENT - THIS PROGRAM PROVIDES SKILLED SERVICE INCLUDING NURSING, PHYSICAL THERAPY, OCCUPATIONAL THERAPY AND SPEECH THERAPY TO MEDICARE RECIPIENTS WHO MEET THE CONDITIONS OF PARTICIPATION FOR HOME BOUND SERVICES. THIS MEDICARE CERTIFIED PROGRAM IS STRICTLY MONITORED BY CMS (CENTER FOR MEDICAID AND MEDICARE SERVICES) AND A FISCAL INTERMEDIARY (IN THIS CASE PALMETTO GBA) UNDER STRICT FEDERAL GUIDELINES.
