CHARLOTTE COMMUNITY HEALTH CLINIC, INC. IS A NONPROFIT ENTITY PROVIDING COMPREHENSIVE HEALTH CARE SERVICES TO OVER 5,217 CHILDREN AND ADULTS IN THE GREATER CHARLOTTE AREA. WE OFFER PRIMARY, DENTAL, AND INTEGRATED BEHAVIORAL HEALTH SERVICES. ALL PATIENTS CAN APPLY FOR OUR SLIDING SCALE FEE TO ENSURE THAT SERVICES ARE AFFORDABLE AND ACCESSIBLE, AS WE HAVE HISTORICALLY SERVED LOW-INCOME AND OTHER UNDERSERVED INDIVIDUALS. ADDITIONAL SERVICES OFFERED INCLUDE ACUTE CARE, CHRONIC DISEASE MANAGEMENT, PREVENTIVE HEALTH, COVID VACCINES, HIV TESTING AND COUNSELING, AND HEALTH EDUCATION. (CONTINUED ON SCHEDULE O)DURING 2021, WE PILOTED THE PRIMARY CARE BEHAVIORAL HEALTH (PCBH) MODEL IN WHICH PRIMARY CARE PROVIDERS AND LICENSED CLINICAL SOCIAL WORKERS WORK TOGETHER TO TREAT BEHAVIORAL HEALTH CONCERNS AS THEY ARISE DURING PRIMARY CARE VISITS. IT HAS PROVEN TO BE A MORE ACCESSIBLE AND EFFICIENT MANNER OF PROVIDING BEHAVIORAL HEALTH SERVICES TO OUR PATIENT POPULATION. OUR CARE MANAGEMENT TEAM ALSO EXPANDED TO INCLUDE TWO SOCIAL WORKERS, WITH ONE SPECIALIZING IN HIV CARE AND PREVENTION. WE ALSO HIRED AN INSURANCE NAVIGATOR / PARALEGAL TO ASSIST OUR PATIENTS WHO MAY QUALIFY FOR BENEFITS AND OTHER SPECIAL PROGRAMS. OUR DENTAL STAFF OFFICIALLY LAUNCHED THEIR SCHOOL-BASED ORAL HEALTH PROGRAM THAT SERVES ELEMENTARY-AGED STUDENTS AT SUGAR CREEK CHARTER SCHOOL WITH DENTAL CLEANINGS AND EDUCATION. THE CLINIC CONTINUES TO GROW AS WE PLAN TO OPEN ADDITIONAL SATELLITE CLINICS IN EAST CHARLOTTE OVER THE NEXT THREE YEARS.
