Part 3 - Line 4a

ACTIVITIES, EVENTS AND CLASSES FOR SENIORS, INCLUDING SPORTS PROGRAMS, TRAVEL PROGRAMS, SOCIAL EVENTS AND DANCES. ALSO INCLUDES ACTIVE LIVING EXPO, RESOURCE FAIR WITH 75 BOOTHS OFFERING EDUCATION, INFORMATION, PRODUCTS AND SERVICES FOR SENIORS. ALSO PROVIDES "OUTLOOK ON ACTIVE AGING" MONTHLY NEWSLETTER AND WEBSITE.


OUTREACH SERVICES INCLUDING WORKING WITH SENIORS AND THEIR FAMILIES TO PROVIDE CARE MANAGEMENT, FACILITATE INDEPENDENT LIVING AND ASSIST WITH LIVING TRANSITIONS THROUGH PROGRAMS WHICH PROVIDE MEALS DIRECTLY TO SENIORS' HOMES AND OFFER OTHER HELP SERVICES AT NO COST TO THE SENIOR. ALSO HAS HOMELESS PREVENTION PROGRAM WHICH OFFERS ASSISTANCE AND EMERGENCY RAPID REHOUSING PROGRAM FOR SENIORS WHO ARE IN DANGER OF OR ACTUALLY DO BECOME HOMELESS. HBCOA CONDUCTED 971 HOME VISITS, 504 OFFICE CONSULTATIONS,MADE OVER 12,000 PHONE CALLS,LENT OUT 178 PIECES OF DURABLE MEDICAL AND MOBILITY EQUIPMENT AND PROVIDED CARE MANAGEMENT SERVICES TO A TOTAL OF 289 SENIORS. GRANT TO THE CITY OF HUNTINGTON BEACH, CA, TO COVER THE EXPENSE OF MEALS TO HOME ASSISTANT.


OFFER TRANSPORTATION SERVICES FOR SENIORS PROVIDING DOOR TO DOOR SERVICE TO MEDICAL APPOINTMENTS, SHOPPING, SENIOR CENTER AND OTHER LOCATIONS WITHIN THE CITY OF HUNTINGTON BEACH, CALIFORNIA. THERE IS NO COST TO THE RIDER FOR THE SERVICE, BUT DONATIONS ARE ACCEPTED.
