WE PROVIDE HEALTHCARE AND HEALTH MANAGEMENT SERVICES IN THE OAKLAND, CALIFORNIA BAY AREA. WE SEEK TO RECOGNIZE, EMPATHIZE, AND CORRECTLY RESPOND TO CLIENTS INDIVIDUAL NEEDS BOTH MEDICALLY AND PSYCHOSOCIALLY. EVERYONE IN OUR CARE IS TREATED WITH DIGNITY AND HUMANITY.

SHELTER CLINIC, AIDS DRUGS ASSISTANCE PROGRAM (ADAP), CHRONIC CARE MANAGEMENT PROGRAM (CCM), AIDS MEDI-CAL WAIVER PROGRAM (MCWP), RYAN WHITE HOME AND COMMUNITY BASED HEALTH SERVICES (RW HCBHS) AND 340B DRUG PRICING PROGRAM. SHELTER CLINIC IS WHERE WE OPEN A POP-UP CLINIC TO SERVE THE HOMELESS POPULATION LIVING IN BERKELEY, WHICH OCCURRED ONCE A WEEK. ADAP IS A CONTRACT GIVEN TO US TO HELP US AID PEOPLE LIVING WITH HIV TO SIGN UP FOR IT AND LOWER THEIR MEDICATION COSTS. CCM IS WHERE WE CALL PATIENTS LIVING WITH CHRONIC DISEASES EVERY MONTH, CHECKING IN WITH THEM TO SEE IF THEY ARE ADHERING TO THEIR MEDICATIONS AND TO ASSIST THEM WITH THEIR NEEDS. MCWP IS A COMPREHENSIVE MEDICAL CASE MANAGEMENT PROGRAM WHERE WE ASSIST PLWH FOR EVERY DAY, MEDICAL, AND PSYCHOSOCIAL NEEDS. RW HCBHS PROVIDES HOME HEALTH SERVICES SUCH AS PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH THERAPY, SKILLED NURSING, AND HOME HEALTH AIDE SERVICES. 340B DRUG PRICING PROGRAM PROVIDES AFFORDABLE SPECIALTY DRUGS TO PLWH.
