SELF DIRECT: PROGRAM TO SUPPORT AND ENABLE INDIVIDUALS WITH DEVELOPMENTAL DISABILITIES TO SELF-DIRECT THEIR SERVICES AND SUPPORTS USING A PERSON-CENTERED PLANNING MODEL. INDIVIDUALS WILL RECEIVE HOME AND COMMUNITY BASED SERVICES AND SUPPORTS THROUGH A PERSON-CENTERED SELF-DIRECTED PLAN OF CARE.
