SHARED LIVING PROGRAM - NONOTUCK RESOURCE ASSOCIATES MATCHES QUALIFIED CAREGIVERS WITH PEOPLE WHO HAVE INTELLECTUAL, PHYSICAL AND/OR OTHER DISABILITIES TO LIVE TOGETHER IN EITHER THE HOME OF THE CAREGIVER OR THE HOME OF THE PERSON RECEIVING THE SERVICE. THE SHARED LIVING CAREGIVERS PROVIDE ON-GOING SUPPORT IN THE ACTIVITIES OF DAILY LIVING, MAINTAINING OPTIMAL HEALTH, RELATIONSHIP DEVELOPMENT, COMMUNITY INCLUSION AND ANY OTHER NEEDED SUPPORT IDENTIFIED IN THE PERSON'S INDIVIDUAL SERVICE PLAN.340 INDIVIDUALS RECEIVED SERVICES.

THIS PROGRAM IS DESIGNED TO SUPPORT FAMILIES TO REMAIN INTACT AND TO KEEP PEOPLE OUT OF INSTITUTIONAL CARE. 914 INDIVIDUALS RECEIVED SERVICES.
COMMUNITY BASED DAY SERVICES - THE INDIVIDUALIZED NATURE OF THIS SERVICE ALLOWS NONOTUCK RESOURCE ASSOCIATES TO DEVELOP RELEVANT, COMMUNITY-BASED PROGRAMS FOR THE PARTICIPANTS. EACH PERSON EXPANDS HIS OR HER CIRCLE OF RELATIONSHIPS, INCREASES THEIR NATURAL INVOLVEMENT IN THEIR COMMUNITY AND EXPANDS THEIR VOCATIONAL AND GENERAL LIFE SKILLS. THROUGH PERSONAL CHOICE, PARTICIPANTS EXPERIENCE THE DIGNITY OF RISK AND EXERCISE THEIR VOICE. PEOPLE WORK, VOLUNTEER, AND PURSUE HOBBIES THAT BUILD SKILLS, PERSONAL RELATIONSHIPS AND SELF-ESTEEM. 8 INDIVIDUALS RECEIVED SERVICES.
