CURATIVE CONNECTIONS HELPS PEOPLE ACHIEVE THEIR GOALS FOR INDEPENDENCE, INCLUDING PEOPLE WITH COGNITIVE AND PHYSICAL DISABILITIES, PEOPLE WITH NEUROLOGICAL IMPAIRMENTS, OLDER ADULTS, PEOPLE WITH DEMENTIA, AND YOUTH AND ADULTS WITH MENTAL HEALTH CONCERNS. PROGRAMS INCLUDE ADULT DAY CARE (INCLUDING DEMENTIA-SPECIFIC SERVICES), EMPLOYMENT AND TRAINING SERVICES, SUPPORTIVE SERVICES BOTH IN THE HOME AND ELSEWHERE IN THE COMMUNITY, SPECIALIZED LEARNING PROGRAMS, AND SPECIALIZED TRANSPORTATION SERVICES. EMPLOYMENT & TRAINING SERVED 492 PEOPLE IN 2021 CURATIVE CONNECTIONS HELPS PEOPLE OF ALL ABILITIES OVERCOME BARRIERS TO EMPLOYMENT AND OBTAIN JOBS IN THE COMMUNITY. WE OFFER TRAINING PROGRAMS WITH HANDS-ON WORK EXPERIENCES IN A VARIETY OF SETTINGS AND WORK PLACES IN THE COMMUNITY. WE CAN HELP PEOPLE CREATE JOB PREP PLANS THAT INCLUDE RESUME BUILDING, JOB SEARCHING AND PLACEMENT ASSESMENTS. WE CAN ALSO PROVIDE JOB COACHING AT PLACE OF EMPLOYMENT TO ENSURE SUCCESSFUL EMPLOYMENT. WE ALSO OPERATE A FEDERAL GRANT THAT OFFERS PEOPLE AGE 55 AND OLDER WHO HAVE LOWER INCOMES A PAID, ON-THE-JOB TRAINING ASSIGNMENT. THE GOAL OF THE 14-COUNTY PROGRAM, CALLED SENIOR TRAINING AND EMPLOYMENT PROGRAM (STEP), IS TO HELP PARTICIPANTS EVENTUALLY SECURE PERMANENT EMPLOYMENT. THE STEP PROGRAM IS OFFERED IN BROWN, CALUMET, DOOR, KEWAUNEE, LINCOLN, MANITOWOC, MARATHON, MENOMINEE, OUTAGAMIE, OZAUKEE, SHAWANO, SHEBOYGAN, WAUPACA AND WINNEBAGO COUNTIES.ADULT DAY SERVICES OFFER DAY PROGRAM OPTIONS FOR OLDER ADULTS AND PEOPLE WITH DISABILITES THAT BENEFIT FROM A SUPPORTIVE SOCIAL SETTING TO ENGAGE WITH OTHERS, FIND MEANING AND PURPOSE IN THEIR DAYS, AND CONTINUE TO BE A PART OF THEIR COMMUNITY. WITH THE GOAL OF HELPING PEOPLE MAINTAINING THEIR COMMUNITY LIVING SITUATION THROUGH SUPPORTIVE SERVICES. 2021 - SERVED 382 PEOPLE; HOURS OF OPERATIONS: MONDAY - FRIDAY 7AM-5:30PM. ALL PROGRAMS ARE CERTIFIED THROUGH THE STATE OF WISCONSIN. IN 2021 TRANSPORTATION SERVICES PROVIDED 42,286 DISTINCT TRIPS THROUGH BROWN COUNTY FOR PEOPLE WITH DISABILITIES AND VULNERABLE OLDER ADULTS, TRAVELING 510,930 MILES. THE PROGRAM USES 40 VOLUNTEERS AND 25 VEHICLES TO TRANSPORT PEOPLE TO AND FROM A VARIETY OF APPOINTMENTS IN THE COMMUNITY TO HELP THEM REMAIN CONNECTED, SAFE AND HEALTHY. 2021 - SERVED 906 PEOPLE.
