THE ORGANIZATION COMPLETED 1,646 HOLISTIC, COMPREHENSIVE ASSESSMENTS FOR OLDER ADULTS AND THEIR FAMILIES DURING FY21. BASED ON THE NEEDS OF THE OLDER ADULT, ORGANIZATION STAFF MADE THE APPROPRIATE REFERRALS TO STATE AND FEDERALLY SUBSIDIZED PROGRAMMING TO ASSIST THE OLDER ADULT IN REMAINING IN THE COMMUNITY AS LONG AS AND AS SAFELY AS POSSIBLE.
TITLE III-B, UNDER THE OLDER AMERICANS ACT, ASSISTS OLDER ADULTS THAT REQUIRE ASSISTANCE WITH AT LEAST ONE ACTIVITY OF DAILY LIVING OR INSTRUMENTAL ACTIVITY OF DAILY LIVING WITH IN-HOME SERVICES. THE ORGANIZATION PROVIDED 4,120 UNITS OF SERVICES DURING FY21. THE ORGANIZATION IS ALSO A PART OF THE AGING DISABILITY RESOURCE NETWORK (ADRN), WHICH PROVIDES INFORMATION, ASSISTANCE, AND REFERRALS TO MEET THE EXPRESSED NEED. AS AN ADRN, DURING FY21, THE ORGANIZATION SERVED 26,849 PEOPLE.
HOME DELIVERED MEALS ARE PROVIDED TO INDIVIDUALS 60 YEARS OF AGE AND OVER WHO ARE EITHER TEMPORARILY OR PERMANENTLY UNABLE TO MANAGE MEAL PREPARATION AND ACCESS FOOD. THE NUTRITIOUS HOMEDELIVERED MEAL SERVICE DOUBLES AS A WELLBEING CHECK NECESSARY FOR INDIVIDUALS WHO EXPERIENCE SOCIAL ISOLATION. DURING FY21, THE ORGANIZATION DELIVERED A COMBINED 50,388 MEALS; HOT, COLD, AND FROZEN. ADDITIONALLY, THE ORGANIZATION DELIVERED 6,704 SPECIAL DIET MEALS AND 2,390 SHELF STABLE MEALS.
VARIOUS OTHER PROGRAMS FOR THE ELDERLY, INCLUDING ADULT PROTECTIVE SERVICES AND CAREGIVER AND CAREGIVER RESPITE SERVICES.
