SUPPORTIVE SERVICES: PROVIDE TRANSPORTATION TO MEDICAL APPOINTMENTS, SENIOR CENTER ACTIVITIES, OR NECESSARY SHOPPING; OPTIONS COUNSELING, CARE MANAGEMENT, IN-HOME SERVICES, TELEPHONE REASSURANCE, FRIENDLY VISITING AND MEDICAL ALERT SYSTEMS SO THAT PEOPLE CAN REMAIN IN THEIR OWN HOMES; INFORMATION AND CONNECTION TO A WIDE VARIETY OF SERVICES AVAILABLE IN THE COMMUNITY; AND LIMITED LEGAL SERVICES.
NUTRITION: PROVIDE HOT, NUTRITIOUS MEALS IN DINING ROOMS WEEKDAYS AT LUNCHTIME; HOT AND FROZEN MEALS DELIVERED TO SENIORS WHO ARE HOMEBOUND - ALL TO HELP PERSON AVOID ISOLATION, BE WELL NOURISHED, AND REMAIN INDEPENDENT.
