NUTRITION: PROVIDE HOT, NUTRITIOUS MEALS IN DINING ROOMS WEEKDAYS AT LUNCHTIME; HOT AND FROZEN MEALS DELIVERED TO SENIORS WHO ARE HOMEBOUND - ALL TO HELP PERSON AVOID ISOLATION, BE WELL NOURISHED, AND REMAIN INDEPENDENT.
DISEASE PREVENTION, FAMILY CAREGIVER, AND OMBUDSMAN SERVICES: PROVIDE A WIDE RANGE OF SERVICES THAT PROMOTE PHYSICAL AND MENTAL HEALTH AND FITNESS USING EVIDENCE-BASED ACTIVITIES; EXERCISE CLASSES, HEALTH SCREENINGS, SOCIAL ACTIVITIES; HELP IN SELF-MANAGEMENT OF DISEASES SUCH AS DIABETES AND HIGH BLOOD PRESSURE; MEDICATION MANAGEMENT; OMBUDSMAN SERVICES TO ADVOCATE FOR PEOPLE WHO LIVE IN NURSING HOMES AND TO PREVENT ELDER ABUSE; AND FAMILY CAREGIVER SERVICES FOR THOSE PROVIDING CARE.
