FUNDED 35 SENIOR CENTER OPERATIONS INCLUDING NOON DAY MEALS SERVED IN A CONGREGATE SETTING AND HOME DELIVERED MEALS. HAD VOLUNTEER HOURS AMOUNTING TO $919,490.
SUPPORTIVE SERVICES PROVIDED ON A CONTRACTUAL AND DIRECT SERVICES BASIS, INCLUDING TRANSPORTATION, INFORMATION & ASSISTANCE, CASE MANAGEMENT, PUBLIC INFO AND EDUCATION, HOMEMAKER, PERSONAL CARE, LEGAL ASSISTANCE, FALL PREVENTION, IN-HOME SAFETY ASSESSMENTS, EYECARE, PERSONAL EMERGENCY RESPONSE, HEALTH SCREENING, AND TELEPHONE REASSURANCE.
