VIA CHRISTI HEALTHCARE OUTREACH PROGRAM FOR ELDERS, INC, (HOPE), INSPIRED BY THE HEALING MISSION OF THE CATHOLIC CHURCH TO THE ELDERLY, PROVIDES IN-HOME AND COMMUNITY SERVICES TO ELDERLY AND DISABLED PERSONS. VIA CHRISTI HOPE, INC. OFFERS THE PACE PROGRAM (ALL-INCLUSIVE CARE FOR THE ELDERLY), WHICH IS AN ALTERNATIVE TO NURSING HOME CARE FOR THOSE WHO REQUIRE LONG-TERM HEALTH CARE BUT WISH TO REMAIN LIVING IN THEIR OWN HOME. THE PROGRAM HELPS PEOPLE LIVE AT HOME, RECEIVE LONG TERM CARE SERVICES AND STILL REMAIN PART OF THEIR COMMUNITY. VIA CHRISTI HOPE, INC. SERVES PEOPLE IN THE WICHITA AREA, AND THE PRIMARY COUNTIES INCLUDE SEDGWICK AND BUTLER. A TREND OF SIGNIFICANCE IN THE COMMUNITY IS THE GROWING ELDERLY POPULATION, TO ADDRESS THIS TREND, VIA CHRISTI HOPE, INC, IS COLLABORATING WITH OTHER COMMUNITY ORGANIZATIONS, INCLUDING BUT NOT LIMITED TO: MEALS ON WHEELS AND NURSING EDUCATION ORGANIZATIONS TO PROVIDE SERVICES AND EDUCATION TO THE ELDERLY.
