PROVIDE SERVICES UNDER TITLE III OF THE OLDER AMERICAN CITIZENS ACT OF 1965. THE ASHLAND COUNTY COUNCIL ON AGING, INC. (THE AGENCY) IS A PRIVATE, NONPROFIT ORGANIZATION ESTABLISHED TO AID PERSONS 60 YEARS OF AGE AND OLDER IN THEIR EFFORTS TO LIVE INDEPENDENTLY.

TITLE III B - PROVIDES FUNDING FOR AN ASSORTMENT OF SOCIAL SERVICE PROGRAMS FOR PERSONS AGE 60 AND OVER.
TITLE III C1 - PROVIDES FUNDING FOR CONGREGATE MEAL PROGRAMS FOR PERSONS OVER 59 YEARS OF AGE. FUNDS ARE USED TO PURCHASE MEALS AS WELL AS SUPPORT PROGRAM OPERATIONS. TITLE III C2 - PROVIDES FUNDING FOR HOME-DELIVERED MEALS TARGETING FRAIL AND IMPAIRED PERSONS OVER 59 YEARS OF AGE. FUNDS ARE USED TO PURCHASE MEALS AS WELL AS SUPPORT PROGRAM OPERATIONS.
PASSPORT (PRE-ADMISSION SCREENING SYSTEM PROVIDING OPTIONS AND RESOURCES TODAY) PROVIDES FAMILIES AN ATTRACTIVE ALTERNATIVE TO TRADITIONAL NURSING HOME PLACEMENT BY PROVIDING ESSENTIAL IN-HOME SERVICES. PASSPORT IS A MEDICAID WAIVER PROGRAM THAT PROVIDES SCREENING, ASSESSMENT, INFORMATION AND ASSISTANCE, CASE MANAGEMENT AND IN-HOME SERVICES TO ELIGIBLE LOW-INCOME FRAIL OLDER ADULTS.
